摘 要:目的:对北京市某三级医院实施的主诊医师负责制进行效果评价,探讨主诊医师负责制实施的可行性。方法:采用历史资料对照比较法,对北京市某试点医院实施主诊医师负责制前后的医疗指标进行描述性统计分析;采用数据包络分析法进行效率评价;采用倍差法进行多因素相关性分析。结果:主诊医师负责制实施后,该医院医疗业务量、医疗质量、医疗效率与效益均得到提高,但试点科室的改善程度不具有显著优势,改革效果与多因素的相关性不强(均P>0.05)。结论:主诊医师负责制在三级医院的实施具有一定的可行性和前景价值,但仍需进一步处理好科室主任权力分配与领导地位的关系,完善人事岗位设置、绩效考核体系、医院信息化建设等配套政策及设施。Objective: To evaluate the effect of responsibility system of chief attending physician in a tertiary hospital of Beijing, and explore the feasibility of the responsibility system of chief attending physician in tertiary hospitals and regions across the country. Methods: Historical data contrast method was used to do descriptive analysis on the changes of medical indicators before and after the trial of responsibility system of chief attending physician by using. Data envelopment analysis (DEA) was used to evaluate the efficiency. Difference in differences (DID) was used to do multi factor correlation analysis. Resnlts: After the implementation of the responsibility system of chief attending physician, overall hospital medical business, medical quality, medical efficiency and the effectiveness were all improved. But to a certain degree, the advantage of trial department was not statistically significant. The correlation between reform effect and multiple factors was not tight. Conclusion: The responsibility system of chief attending physician in tertiary hospital had certain feasibility and prospects. However, it has to further handle the relationship between power distribution and leading position of the heads of each department, and to consummate personnel post setting, performance evaluation system, hospital information construction and other supporting policies and facilities.
