摘 要:目的了解阿尔茨海默病并发尿路感染患者尿液中分离的病原菌分布及其耐药性。方法回顾分析本地区阿尔茨海默病并发尿路感染患者尿液培养检出细菌的分布和耐药情况。结果 394例患者培养出细菌244株,总阳性率为61.9%,其中革兰阴性菌137株,革兰阳性菌64株,真菌43株。革兰阴性菌对碳青霉烯类药物(亚胺培南、美罗培南)完全不耐药;对阿米卡星与头孢哌酮/舒巴坦的耐药率较低。大肠埃希菌ESBLs的阳性率最高(52.4%),其次为奇异变形杆菌(31.3%)、阴沟肠杆菌(17.6%)。革兰阳性菌中的粪肠球菌、屎肠球菌和凝固酶阴性葡萄球菌均未检测到万古霉素和利奈唑胺的耐药性;粪肠球菌和屎肠球菌的高浓度庆霉素耐药率分别为20.8%和66.7%。结论阿尔茨海默病并发尿路感染患者尿液分离菌以大肠埃希菌为主,其次为肠球菌,真菌以白色念珠菌为主。了解本群患者的尿路分离菌菌谱及耐药性,将有助于抗菌药物的合理选用,提高疗效。Objective To investigate the pathogens distribution and drug resistance in Alzheimer patients with urinary tract infection. Methods Pathogen distribution and drug resistance in urine from Alzheimer patients with urinary tract infection were retrospectively analyzed. Results 244 pathogens were isolated from 394 patients,with the total positive rate of 61. 9%. Among them,137 strains were gram-negative,64 strains were gram-positive and 43 strains were fungi. Gram-negative bacteria were all susceptible to imipenem and meropenem,while the resistance to amikacin and cefoperazone/sulbactam were lower respectively. The positive rates of ESBLs of Escherichia coli,Proteus mirabilis,Enterobacter cloacae were 52. 4%,31. 3%,17. 6%. Gram-positive bacteria including Enterococcus faecalis,Enterococcus faecium and coagulase-negative staphylococcus were all susceptible to vancomycin and linezolin. The drug resistance of Enterococcus faecalis and Enterococcus faecium to HLGR were 20. 8% and 66. 7%. Conclusion The main pathogens was Escherichia coli in the urine from patients with Alzheimer complicated with urinary tract infection,followed by Enterococcus faecium,with Candida albicans the dominant in fungi. To understand the isolates distribution and drug resistance will help the rational choice of antibiotics and improve the curative effect.
