以视野缺损及视力下降为首发症状的脑血管病临床分析

摘　　要：目的探讨以视野缺损及视力下降为首发症状的脑血管病的理论依据。方法选取2014年2月至2015年9月以视野缺损、视力下降为首发症状的脑血管疾病患者15例，回顾性分析其临床病史资料，主要包括临床诊断结果、人口学特征、既往病史、视功能损害及眼底表现。结果15例患者中出血性脑血管病1例，缺血性脑血管病14例；前循环4例，后循环11例。既往病史中血脂异常、糖尿病及高血压病发病率分别为80．00％、66．67％、53．33％，而仅有33．33％的患者有脑血管病史。患者视功能损害及眼底表现主要包括突发双眼视物不见、双眼视物模糊、双眼先后视力下降、急性单眼视力下降。结论单眼症状属前循环病变，由闭塞及颈内动脉缺血引发，而双眼共同偏盲或发病多由后循环疾病损伤枕颞叶所致。Objective To investigate the theoretical base of cerebrovascular disease with visual field defect and hypopsia as the first symptoms. Methods From February 2014 to September 2015, 15 patients with cerebral vascular disease were selected, with visual field defect and hypopsia as the first symptoms, the clinical data of the patients were retrospectively analyzed, including the clinical diagnosis, demographic characteristics, past medical history, visual function damage and fundus manifestations. Results Among the 15 patients, hemorrhagic cerebrovascular disease in 1 case, 14 cases of ischemic cerebrovascular disease; 4 cases of anterior circulation, posterior circulation in 11 cases. The past history of dyslipidemia, diabetes and hypertension incidence rates were 80. 00%, 66. 67%, 53.33%. While only 33.33% of patients with previous history of cerebrovascular disease. Patients with visual impairment and fundus manifestations including sudden binocular vision was gone, eyes blurred vision, binocular vision decreased, acute monocular vision loss. Conclusions Monocular symptom is lesions in anterior cir- culation, ischemia caused by occlusion of internal carotid artery, and the eyes together are blind or morbidity caused by circulatory diseases injuries after occipital lobe.
