小剂量顺铂与洛铂联合依托泊苷一线治疗老年广泛期小细胞肺癌的临床研究

摘　　要：目的评价小剂量顺铂与标准剂量洛铂联合依托泊苷一线治疗老年广泛期小细胞肺癌的疗效及副作用。方法回顾性分析初治的老年广泛期小细胞肺癌32例,根据化疗方案不同分为两组,EL(依托泊苷+洛铂)组18例,EP(依托泊苷+顺铂)组14例,治疗21 d为1周期,至少2周期后评价疗效及副作用。结果 EL组与EP组有效率差别无统计学意义(P>0.05);中位无进展生存时间(PFS)均为8月(P>0.05);两组患者消化道反应(88.89%vs.100.00%),白细胞减少(44.44%vs.71.43%)及贫血发生率(44.44%vs.57.14%)差别均无统计学意义(P>0.05),两组均未发生血小板减少,两组均未见肝肾功能损害。结论小剂量顺铂与标准剂量洛铂联合依托泊苷治疗老年广泛期小细胞肺癌的疗效及副作用相当,降低顺铂的剂量强度在降低消化道反应的同时并未降低疗效,两者均可作为老年广泛期小细胞肺癌的一线用药。Objective To evaluate the efficacy and adverse effects on lower dose of cisplatin versus standard dose of lobaplatin combined with etoposide on first-line treatmenting elderly patients of extensive stage small cell lung cancer. Methods The research retrospectively analysed 32 elderly patients of extensive stage small cell lung cancer, and the patients were divided into EL （etoposide＋lobaplatin） group of 18 cases and EP （etoposide＋cisplatin） group of 14 cases according to different chemotherapy regimens. Every treatment cycle had 21 d, and the efficacy and adverse effects were evaluated after 2 or more cycles. Results In EL and EP group, there were no statistical significant difference in response rate （RR） （P〉0.05）; the median progression free survival were 8 months in both two groups （P〉0.05）; EL and EP group did not significantly differ in incidence of gastrointestinal reaction （88.89% vs. 100.00%, P〉0.05）, leukopenia （44.44% vs. 71.43%, P〉0.05）, and anemia （44.44% vs. 57.14%, P〉0.05）, the two groups both had no thrombocytopenia, and liver and kidney function damage. Conclusion Lower dose of cisplatin and standard dose of lobaplatin plus etoposide regimes have similar efficacy and adverse effects in the first line treatment on elderly patients of extensive stage small cell lung cancer. The reduction of the dose intensity of cisplatin can lower digestive tract reaction and do not induce the poorer efficacy and both the two medicine can be the first line drugs for elderly patients of extensive stage small cell lung cancer.
