多发性骨髓瘤病人血清SFRP3检测及其临床意义

摘　　要：目的探讨多发性骨髓瘤（MM）病人血清分泌型卷曲相关蛋白3（SFRP3）的表达及临床意义。方法收集2007年7月—2014年7月于聊城市人民医院血液科住院的初治MM病人70例,用酶联免疫吸附试验法检测血清SFRP3水平,并分析其与骨髓瘤骨病（MBD）、疗效和生存的关系。结果 MBD 0~1级病人血清SFRP3水平低于2~3级和4级病人,MBD 2~3级病人血清SFRP3水平低于4级病人,差异均有显著意义（F=20.89,P〈0.01）。MM病人治疗后,获得完全缓解（CR）病人的血清SFRP3水平较治疗前明显降低（t=3.76,P〈0.01）;获得部分缓解（PR）病人的血清SFRP3水平较治疗前下降,达不到PR病人的血清SFRP3水平较治疗前上升,但差异均无显著性（P〉0.05）。血清SFRP3水平≤0.65μg/L的病人中位生存期较〉0.65μg/L的病人长,但差异无显著性（P〉0.05）。结论 MM病人血清SFRP3水平与MBD和疗效相关,对疗效和预后有提示作用。Objective To investigate the expression of serum secreted frizzled related protein 3（SFRP3）and its significance in patients with multiple myeloma（MM）. Methods Seventy patients who received initial treatment for MM-from July2007 to July 2014 in the Department of Hematology of Liaochen People＇s Hospital-were selected.Employing enzyme-linked immunoadsordent assay technique,the levels of SFRP3 were detected,and the relationship among myeloma bone disease（MBD）,efficacy and survival was analyzed. Results Serum levels of SFRP3 in MBD grade 0-1patients were lower than that in MBD grade 2-4ones,and that in grade 2-3patients were lower than that in grade 4ones,the difference was significant（F=20.89,P〈0.01）.After treatment,the serum levels of SFRP3 in patients with complete remission notably declined versus that before treatment（t=3.76,P〈0.01）.In patients with partial remission,the levels of SFRP3 declined,but the difference in that versus before treatment was not significant.The median survival time in patients with serum level of SFRP3≤0.65μg/L was longer than those with 〉0.65μg/L,but the difference was not significant. Conclusion Serum SFRP3 level in patients with multiple myeloma is associated with myeloma bone disease and its efficacy,which predicts therapeutic effectiveness and patient＇s prognosis.
