摘  要:目的 miRNA-135a作为miRNA-135家族的重要组成成员,其与多种肿瘤发生发展密切相关。但关于miRNA-135a的表达异常对结肠癌的临床意义及预后判断的研究较少。本研究旨在明确结肠癌组织miRNA-135a相对表达量与结肠癌患者临床病理因素的相关性及对患者生存预后的影响。方法收集2009-01-01-2009-12-31在哈尔滨医科大学附属第四医院行手术切除的结肠癌组织样本及距离肿瘤边缘5cm处癌旁黏膜组织样本共47对。在47对结肠癌样本中应用逆转录实时定量聚合酶链反应,检测miRNA-135a相对表达量,进一步分析其表达变化与结肠癌临床病理特征的相关性。并运用Kaplan-Meier生存曲线进行预后分析。结果 miRNA-135a在结肠癌组织中表达较癌旁组织显著升高,P=0.03。miRNA-135a表达量变化与患者年龄、性别、肿瘤大小、肿瘤部位、肿瘤分化程度、浸润深度及远处转移均无关,P值均>0.05。但在淋巴结转移组miRNA-135a表达明显升高,均值为0.49±0.25,而无淋巴结转移组为0.35±0.17,两组比较差异有统计学意义,P=0.03。在临床Ⅲ/Ⅳ期的患者中miRNA-135a表达较Ⅰ/Ⅱ期的患者显著升高,两组比较差异有统计学意义,P=0.04。Logistic二项回归分析显示,miRNA-135a高表达组增加了结肠癌患者淋巴结转移的风险,P=0.03。miRNA-135a高表达同时可增加结肠癌疾病进展(TNM分期)的风险,P=0.04。生存分析表明,21例miRNA-135a高表达组的中位生存期为(41±9.73)个月,26例miRNA-135a低表达组的中位生存期为(58±6.16)个月,两组比较差异有统计学意义,P=0.021。结论 miRNA-135a表达量增高与结肠癌疾病进展及不良预后正相关。miRNA-135a有望成为判断结肠癌预后的潜在靶点。OBJECTIVE miRNA-135a, which belongs to miRNA-135 super family, plays important roles in the development and progression of many cancers. However, the role of miRNA-135a in the clinical significance and progno- sis of colon cancer is still unknown. This study aims to investigate the relative expression levels of miRNA-135a in colon cancer, the relation between miRNA-135a expression and clinicopathological features and prognosis of colon cancer. METHODS Forty-seven pairs of colon cancer tissues and adjacent normal tissues were obtained from the patients who had undergone resection between January 1, 2009 and December 31, 2009 in the Bio-Bank of Department of General Sur-gery, the Forth Affiliated Hospital of Harbin Medical University. The relative expression of miRNA-135a was measured by quantitative real-time PCR (qRT-PCR). The miRNA-135a expression levels and the clinicopathological characteristics of colon cancer were further analyzed. Prognosis was analyzed by the Kaplan-Meier survival curves. RESULTS miRNA- 135a expression levels increase significantly in the colon cancer tissues compared with the non-tumor control tissues(P=0.03). The expression levels of miRNA-135a are not associated with age, gender, tumor size and locations, tumor differ- entiation, depth of tumor invasion and metastasis (P〉0.05). However, in the group of positive lymphatic metastasis, the expression of miRNA-135a is remarkably increased (0. 49±0. 25) compared with the group of none lymphaticmetastasis (0. 35 ± 0. 17). Significant differences are found between positive and negative lymphatic metastasis groups (P=0.03). Meanwhile, miRNA-135a expression levels are higher in stage Ⅲ/Ⅳ than in stage I/lI colon cancer patients (P=0.04). Increased expression of miRNA-135a can increase the risk of lymphatic metastasis (P=0.03) and dis- ease progression (P=0.04) in colon cancer patients by binary logistic regression analysis.
