经乳晕边缘切口乳腺纤维腺瘤切除术中缝闭腺体残腔与保留腺体残腔疗效比较

摘　　要：目的比较经乳晕边缘切口乳腺纤维腺瘤切除术中缝闭腺体残腔与保留腺体残腔的临床疗效。方法选择2011年6月至2013年6月玉田县妇幼保健院收治的乳腺纤维腺瘤患者80例,均给予经乳晕边缘切口乳腺纤维腺瘤切除术治疗,其中40例术中缝闭腺体残腔(缝闭腺体残腔组),40例术中保留腺体残腔(保留腺体残腔组),比较2组患者的手术时间、术后疼痛、切口愈合情况、术后美观度及满意度。结果缝闭腺体残腔组和保留腺体残腔组患者单个瘤体切除时间分别为(15.31±1.98)、(14.86±2.23)min,2组患者的单个瘤体切除时间比较差异无统计学意义(P>0.05);缝闭腺体残腔组和保留腺体残腔组患者切口愈合率分别为100.00%和85.00%,缝闭腺体残腔组患者的切口愈合率显著高于保留腺体残腔组(P<0.05);缝闭腺体残腔组和保留腺体残腔组患者乳房外形变化率分别为2.50%和17.50%,缝闭腺体残腔组患者的乳房外形变化率显著低于保留腺体残腔组(P<0.05)。2组患者术后第1、2天疼痛评分比较差异无统计学意义(P>0.05),但术后第3天缝闭腺体残腔组患者的疼痛评分显著低于保留腺体残腔组(P<0.05)。缝闭腺体残腔组和保留腺体残腔组患者满意度分别为97.50%(39/40)和82.50%(33/40),缝闭腺体残腔组患者满意度显著高于保留腺体残腔组(P<0.05)。结论乳腺纤维腺瘤切除术中缝闭腺体残腔能够有效地减轻患者术后疼痛,有助于切口愈合,且乳腺外形保持较好。Objective To compare the effect between closing gland residual cavity and retaining gland residual cavity in the excision of breast fibroadenoma via periareolar incision. Methods Eighty patients with breast fibroadenoma were selected in Maternal and Child Health Care Hospital of Yutian County from June 2011 to June 2013. All the patients were treated with the excision of breast fibroadenoma via periareolar incision. Among the patients, the gland residual cavities were closed in forty cases （ closing residual cavity group） , and the gland residual cavities were retained in the other forty cases （retaining residual cavity group）. The operation time,postoperative pain,wound healing, postoperative beauty degree and satisfaction degree were compared between the two groups. Results The single tumor resection time in closing residual cavity group and retaining residual cavity group was （ 15.31 ± 1.98 ） and （ 14.86 ± 2.23 ） min respectively, there was no significant difference between the two groups（ P 〉 0.05 ）. The wound healing rate in closing residual cavity group and retaining residual cavity group was 100.00% and 85.00% respectively, the wound healing rate in closing residual cavity group was significantly higher than that in retaining residual cavity group（ P 〈 0.05 ）. The breast shape change rate in closing residual cavity group and retaining residual cavity group was 2.50% and 17.50% respectively ,the breast shape change rate in closing residual cavity group was significantly lower than that in retaining residual cavity group（ P 〈0.05 ）. There was no significant difference in the pain score between the two groups on the first and second day after operation（ P 〉 0.05 ）. But the pain score of patients in closing residual cavity group was significantly lower than that in retaining residual cavity group on the third day after operation（ P 〈 0.05 ）. The satisfaction degree of patients in closing residual cavity group and retaining residual cavity group was 97.50% and 82.
