金华地区Ki-67在乳腺癌中的表达及意义

摘　　要：目的探讨金华地区Ki-67在乳腺癌组织中的表达水平、与乳腺癌亚型的关系及其对患者行改良乳腺癌根治术后局部复发、远处转移的预测价值。方法选取我院行改良乳腺癌根治术治疗的乳腺癌患者134例,术后均随访至少3年。检测其乳腺癌组织Ki-67、ER、PR及Her-2表达水平和癌旁组织Ki-67表达水平,分析乳腺癌Ki-67蛋白表达水平及其与乳腺癌亚型的关系、乳腺癌Ki-67蛋白表达水平与局部复发和远处转移的关系及其预测局部复发和远处转移等预后情况的价值。结果乳腺癌患者乳腺癌组织Ki-67蛋白阳性表达率高于癌旁组织(P<0.05)。Ki-67在金华地区乳腺癌患者不同亚型的蛋白阳性表达率分别为Luminal A型53.57%、Luminal B型77.27%、Her-2阳性型94.29%、三阴型96.30%(P<0.05)。Ki-67在金华地区乳腺癌患者乳腺癌组织蛋白表达水平与乳腺癌亚型密切相关。Ki-67阳性表达患者的局部复发率及远处转移率均高于阴性表达患者(P<0.05)。Ki-67在金华地区乳腺癌患者乳腺癌组织蛋白表达水平与局部复发和远处转移均密切相关,且其预测局部复发和远处转移的价值良好。结论金华地区乳腺癌患者乳腺癌组织Ki-67蛋白表达与乳腺癌亚型和局部复发和远处转移等预后情况均相关,且对预后情况的预测价值良好,可作为乳腺癌亚型和预后评估的参考指标。Objective To investigate the expression level of Ki-67 in breast cancer tissues and its relationship with breast cancer subtypes in Jinhua area, as well as to investigate its predicting value of local recurrence and distant metastasis after modified radical mastectomy. Methods 134 patients with breast cancer who were given modified radical mastectomy were selected, and followed up for at least 3 years after the surgery. The expression levels of K!-67, ER, PR and Her-2 in breast cancer tissues and Ki-67 expression levels in adjacent tissues were detected. The relationship between Ki-67 protein expression and its breast cancer subtypes were analyzed, and the relationship between Ki-67 expression level and its local recurrence and distant metastasis of breast cancer were also analyzed： The value of pre- dicting the prognosis of local recurrence and distant metastasis was analyzed. Results The positive expression rate of Ki-67 protein in breast cancer tissues was significantly higher than that in adjacent tissues （P〈0.05）. The positive ex- pression rates of Ki-67 in different subtypes of breast cancer in Jinhua were Luminal A 53.57%, Luminal B 77.27%, Her-2 positive 94.29%, and triple negative type 96.30%（P〈0.05）. The expression of Ki-67 protein in breast cancer was closely related to breast cancer subtypes in Jinhua area. The local recurrence rate and distant metastasis in the patients with positive expression of Ki-67 were higher than those in the patient with negative expression（P〈0.05）. The expression of Ki-67 protein in breast cancer tissues from Jinhua area was closely related to local recurrence and distant metastasis, and its value for predicting local recurrence and distant metastasis is favorable. Conclusion The expression of Ki-67 protein in breast cancer tissues in the patients with breast cancer from Jinhua area is correlated with the prognosis of breast cancer subtypes and local recurrence and distant metastasis, and its value of predicting the prognosis is favorable, which can be us
