磁性附着体在老年全口覆盖义齿修复中的效果及对牙周健康的影响

摘　　要：目的 研究磁性附着体在老年全口覆盖义齿修复中的效果及对牙周健康的影响,探讨其临床适用性。方法选取2011年4月~2015年10月在我院口腔科就诊的136例牙列缺损老年患者作为研究对象,随机分为对照组和实验组,各68例。对照组实行传统卡环固位体覆盖义齿修复,实验组实行磁性附着体覆盖义齿修复,比较两组治疗后的效果、牙周健康情况及不良反应发生率。结果 实验组治疗后的语言功能、舒适性、义齿外观满意率分别为94.12%、92.65%、97.06%,显著高于对照组的54.41%、41.47%、50.00%,差异有统计学意义（P〈0.01）。实验组治疗后的咀嚼效率和固定力显著高于对照组,差异有统计学意义（P〈0.01）。实验组治疗后的龈沟出血指数、龈沟液量、牙槽骨高度及牙周袋深度等指标均明显优于对照组,差异有统计学意义（P〈0.01）。两组的不良反应发生率均为4.41%,差异无统计学意义（P〉0.05）。结论 磁性附着体在老年全口覆盖义齿修复中的效果显著,患者满意度较高,对牙周损害程度低,值得临床推广应用。Objective To study the effect of magnetic attachment on the restoration of elderly complete overdenture and its influence on periodontal health.Methods 136 elderly patients with dentition defect from April 2011 to October 2015 in the pepartment of dentistry of our hospital were selected and randomly divided into the experience group and the control group,78 cases in each group.The control group was treated with conventional overdenture restoration of card ring retainer, the experience group was treated with overdenture restoration of magnetic attachment.The efficacy,peri- odontal health and the incidence rate of adverse reactions were compared.Results The satisfaction rate of language function,comfort,denture appearance of the experience group was 94.12%,92.65%,97.06% respectively,which was higher than 54.41%,41.47%,50.00% of the control group,with significant difference （P〈0.01）.The chewing efficiency,fixed force of the experience group was higher than that of the control group,with significant difference （P〈0.01）.The indexes of SBI,GCF,alveolar bone height,periodontal pocket depth of the experience group was better than that of the control group,with significant difference （P〈O.01）.The incidence rate of adverse reactions in the two groups were 4.41%,the difference was not statistically significant （P〉0.05）.Conclusion The effect of magnetic attachment on the restoration of elderly complete overdenture is obvious,the satisfaction rate is high,and it hurts little to periodontal health,it is worthy of clinical promotion and application.
