两种不同类型的ICL/TICL治疗近视和散光术后效果比较

摘　　要：目的比较传统型V4-和新型中央孔型V4c-两种类型有晶体眼后房型人工晶体植入治疗中高度近视和散光的效果。方法回顾分析2014年11月至2015年8月在我院行传统型V4-和新型V4c-ICL/TICL植入术的近视和散光患者6个月后的日常视觉功能感受,收集资料完整的病例分别有28例(55眼)和19例(37眼)。记录患者手术前后视力、屈光状态、眼压、角膜内皮细胞等。结果 V4-ICL/TICL型和V4c-ICL/TICL型植入术后6个月,所有患者裸眼视力(Uncorrected visual acuity,UCVA)和最佳矫正视力(best corrected visual acuity,BCVA)均等于或优于术前BCVA;两组间术后UCVA和BCVA差异无统计学意义(P>0.05);有效性和安全性分别是1.19、1.18(P=0.064)和1.20、1.24(P=0.211);分析患者术后远、中、近距离及夜间的视力相关日常活动,所有患者对术后在日常生活中的用眼都感到满意或非常满意。两组间对日常视觉功能评分差异无统计学意义(P>0.05)。结论术后6个月的结果表明不管是V4-ICL/TICL还是V4c-ICL/TICL植入治疗中高度近视和散光,两者都具有良好的有效性和安全性,患者对日常生活中与视觉相关的活动满意度高且二者的评分无统计学意义,说明中央孔的存在不会影响患者的日常视觉功能活动。Objective Comparison of the efficacy of moderate to high myopic and astigmatic patients treated with conventional Implantable Collamer Lens（ V4-ICL/TICL） and new model Implantable Collamer Lens（ V4c-ICL/TICL） respectively. Methods This study evaluated 55 eyes of 28 patients and 37 eyes of 19 patients who had implanted V4-ICL/TICL and V4c-ICL/TICL,respectively. Uncorrected visual acuity（ UCVA） and best corrected visual acuity（ BCVA） distance visual acuities,refraction,intraocular pressure（ IOP）,endothelial cell density,adverse events were recorded. The daily visual functions were evaluated at 6 months after surgery by the same resident doctor. Results At 6 months after implantation,UCVA and BCVA of all eyes were equal or over the pre-operative BCVA; and there had no statistical significance between V4-ICL/TICL and V4c-ICL/TICL（ P= 0. 577,0. 800）. The efficacy index and safety index was 1. 19,1. 18（ P = 0. 064） and 1. 20,1. 24（ P =0. 211） respectively. We found that all patients had satisfaction or high satisfaction about the daily visual functions including distance,middle,near and night vision. There was no significant difference of the score of daily visual functions between V4-ICL/TICL group and V4c-ICL/TICL group（ all P〉0. 05）. Conclusion Our results at 6-month after V4-ICL/TICL and V4c-ICL/TICL implantation showed there had good efficacy and safety,and high satisfaction of daily visual functions,and the score about the daily visual functions between V4-and V4c-ICL/TICL group. It is shown that the central hole of V4c-ICL/TICL did not affect patients＇ daily visual functions.
