机械取栓联合动脉溶栓治疗急性缺血性脑卒中疗效观察

摘　　要：目的探讨应用机械取栓联合动脉溶栓治疗急性脑梗死临床效果及预后情况。方法选取2015年7月至2016年7月应用Solitaire AB支架机械取栓联合动脉溶栓治疗的20例急性脑梗死患者为观察组,同期选取0-6h内发病给予静脉溶栓的脑梗死患者20例为对照组,评价其疗效和预后,对比两组患者治疗前后的美国卫生研究院卒中量表（NIHSS）评分,改良Rankin量表（mRS）评分及不良反应情况。结果观察组与对照组24h、7d、30d、90d NHISS评分比较差异均有统计学意义（P〈0.05）。观察组治疗后30dmRS评分与对照组比较差异有统计学意义（P〈0.05）。观察组与对照组有效率比较差异有统计学意义（P〈0.05）。观察者与对照组不良反应比较差异有统计学意义（P〈0.05）。结论机械取栓联合动脉溶栓治疗急性缺血性卒中对神经功能改善时间、安全性及远期临床预后较好,值得临床推广应用。Objective The clinical effect and prognosis of acute cerebral infarction with the application of MERCI combined with intra-arterial thrombolysis were discussed. Methods 20 patients with acute cerebral infarction treated with Solitaire AB stent MERCI and intra-arterial thrombolysis from July 2015 to July 2016 were selected as the observation group,at the same time 20 cases with sudden attack of cerebral infarction in 0 - 6h given intravenous thrombolysis were selected as control group.Curative effect and prognosis of both groups were evaluated,and National Institute of Health Stroke Scale（ NIHSS） score,modified Rankin Scale（ mRS） score and adverse reactions before and after treatment of two groups of patients were all compared.Results The 24 h,7d,30 d,90d NHISS score of the Observation group and control group had statistically significant difference（ P 0.05）. The 30 d mRS score of the observation group after treatment compared with the control group had statistically significant difference（ P〈0.05）.The effective rate in comparision of both groups had statistically significant difference（ P〈0.05）.The adverse reactionin comparision of both groups had statistically significant difference（ P〈0.05）.Conclusion MERCI combined with intra-arterial thrombolysis for the treatment of acute ischemic stroke is better in improving time,safety and long-term clinical prognosis of neurologic function,so it＇s worthy of clinical application.
