不同剂量低浓度制霉菌素治疗中度鹅口疮患儿的疗效比较

摘　　要：目的探讨不同剂量低浓度制霉菌素治疗中度鹅口疮患儿的临床效果。方法选择2013年2月~2016年2月浙江萧山医院接诊的90例中度鹅口疮患儿,通过随机数表法分为大剂量组（n=45）和小剂量组（n=45）,大剂量组给予20万U/m L的制霉菌素溶液,小剂量组给予10万U/m L制霉菌素溶液。比较2组患儿治疗效果、不良反应和复发情况。结果治疗后3、7 d,2组患儿唾液s Ig A水平增加（P〈0.05）,治疗后15、30 d唾液s Ig A水平有所降低（P〈0.05）,大剂量组治疗3、7、15、30 d时唾液s Ig A水平均比小剂量组高（P〈0.05）;大剂量组临床症状好转时间、临床症状消失时间、实验室检查弱阳性时间、实验室检查阴性时间均低于小剂量组（P〈0.05）;大剂量组治疗总有效率95.56%显著高于小剂量组75.56%（P〈0.05）。治疗过程中,2组患儿不良反应发生率比较差异无统计学意义。随访3个月内,大剂量组复发率2.22%明显低于小剂量组15.56%（P〈0.05）。结论与常规小剂量相比,大剂量低浓度制霉菌素治疗中度鹅口疮患儿效果更加显著,且不良反应较少,复发率低。Objective To study the clinical effect of different doses of low concentrations nystatin on moderate thrush in children. Methods 90 cases of moderate thrush in children from February 2013 to February 2016 in zhejiang xiaoshan hospital were selected,and were divided into the highdose group（ n = 45） and the low-dose group（ n = 45） according to random number table,the high-dose group was treated with 200 thousands U / m L nystatin solution,the low-dose group was treated with 100 thousands U / m L nystatin solution. The treatment effect,adverse reactions and relapse,of two groups were compared. Results On the 3rd and 7th day after treatment,the levels of salivary s Ig A in two groups were increased（ P 0. 05）,the level of s Ig A in saliva decreased at 15 and 30 days after treatment（ P 0. 05）,the levels of s Ig A in saliva at 3,7,15,and 30 days after treatment were significantly higher in the high-dose group than in the low-dose group; The time of improvement of clinical symptoms,the disappearance time of clinical symptoms,the weak positive time of laboratory examination and the negative time of laboratory examination in high dose group were all lower than in lowdose group（ P 0. 05）; the total effective rate in high dose group was 95. 56% which was significantly higher than low dose group 75. 56%（ P 0. 05）.In the process of the treatment,there was no significant difference between the two groups in the incidence of adverse reactions. Follow-up of three months,the recurrence rate of high-dose group was 2. 22%,which was significantly lower than that of low-dose group 15. 56%（ P 0. 05）. Conclusion Compared with the conventional low-dose,high-dose low concentrations nystatin treatment of moderate thrush in children with more significant effect,the adverse reactions and recurrence rate are low.
