清热消炎固齿缓释剂联合中药自拟方治疗慢性牙周炎临床研究  

The clinical effect of Qingre-Xiaoyan-Guchi slow-release agent combined with Chinese medicine in the treatment of chronic periodontitis

摘　　要：目的 评价清热消炎固齿缓释剂联合中药自拟方治疗慢性牙周炎的疗效.方法 将符合入选标准的慢性牙周炎患者98例,采用随机数表法分为2组,每组49例.全部患者均接受常规慢性牙周炎基础治疗,在常规治疗基础上对照组将盐酸米诺环素软膏注入患牙的牙周袋内;观察组将清热消炎固齿缓释剂注入患牙的牙周袋内并口服中药自拟方.2组均治疗3周.观察龈沟出血指数（sulcular bleeding index,SBI）、菌斑指数（plaque index,PLI）、探诊深度（probing depth,PD）、附着丧失（attachment loss,AL）,以及血清IL-6、IL-8、TNF-α水平变化;随访1年,记录复发率.结果 治疗后,观察组SBI[（0.55±0.04）比（1.02±0.07）,t=1.559]、PLI[（1.25±0.03）比（1.36±0.01）,t=1.647]、PD[（2.08±0.25）mm比（3.09±0.11）mm,t=1.574]、AL[（2.80±0.21）mm比（3.89±0.33）mm,t=1.173],以及IL-6[（19.25±11.42）ng/L比（23.76±10.97）ng/L,t=2.066]、IL-8[（2.11±0.45）μg/L比（4.12±0.56）μg/L,t=19.585]、TNF-α[（4.20±1.35）ng/L比（5.11±1.38）ng/L,t=3.230]水平均低于对照组（P〈0.05或P〈0.01）.随访1年,观察组获效的48例患者中复发3例（6.25%）,对照组获效的44例患者中复发12例（27.27%）,2组复发率比较差异有统计学意义（χ2=3.419,P〈0.05）.结论 清热消炎固齿缓释剂联合中药自拟方可降低慢性牙周炎患者炎性因子水平,改善临床症状.Objective To investigate the clinical effect of Qingre-Xiaoyan-Guchislow-release agent combined with Chinese medicine in the treatment of chronic periodontitis.Methods A total of 98 patients with chronic periodontitis were randomly divided into two groups, each group of 49 patients. All the patients received the basic treatment for the chronic periodontitis. The control group injected Palio ointment into the periodontal pocket with the routine treatment. The observation group reveived Qingre-Xiaoyan-Guchi slow-release agent into the periodontal pocket, and took the traditional Chinese medicine prescriptions. Both groups were treated for 3 weeks with 1 year followed-up. The sulcular bleeding index （SBI）, plaque index （PLI）, probing depth （PD）, attachment loss （AL）, IL-6, IL -8, TNF-a levels were assessed to evaluate the clinical effect and recurrence rate. Results After treatment, the observation group SBI （0.55 ± 0.04vs. 1.02 ± 0.07,t= 1.559）, PD （2.08 ± 0.25 mm vs. 3.09 ± 0.11 mm,t=1.574）, AL （2.80 ± 0.21 mmvs.3.89 ± 0.11 mm,t=1.173）, PLI （1.25 ± 0.03vs. 1.36 ± 0.01, t=1.647） were significantly lower than those of the control group （P〈0.05）; the IL-6 （19.25 ± 11.42 ng/Lvs. 23.76 ± 10.97 ng/L,t=2.066）, IL-8 （2.11 ± 0.45μg/L vs. 4.12 ± 0.56μg/L,t=19.585）, and TNF-α （4.20 ± 1.35 ng/L vs. 5.11 ± 1.38 ng/L,t=3.230） level were significantly lower than those in the control group （P〈0.05 orP〈0.01）. Among the 48 patients who were followed up for 1 year, 3 patients （6.25%） relapsed in the observation group and 12 patients （27.27%） relapsed in the control group. There was significant difference on the recurrence rate between two groups （χ2=3.419,P〈0.05）.Conclusions The combination of Qingre-Xiaoyan-Guchi slow-release agent and self-made TCM prescription can reduce the level of inflammatory factors and improve the clinical symptoms.
