摘  要:目的观察急性脑梗死患者脑红蛋白(Ngb)、脑源性神经营养因子(BDNF)和髓鞘碱性蛋白(MBP)水平变化及其与病情严重程度和预后的关系。方法选择我院2012年1月—2015年12月收治的急性脑梗死126例(脑梗死组)及同期在我院体检健康的30例(对照组)。比较两组血清Ngb、BDNF和MBP水平变化,观察各指标与神经功能损伤程度、脑梗死面积和预后的关系及各指标间的相关性。结果与对照组比较,脑梗死组血清Ngb和MBP明显升高(P<0.01),而BDNF则显著降低(P<0.01)。随着神经功能损伤程度的加重和脑梗死面积的增大,脑梗死组血清Ngb和MBP水平升高(P<0.01),BDNF水平降低(P<0.01)。脑梗死组预后良好者Ngb和MBP水平较预后不良者明显降低(P<0.01),而BDNF水平则明显升高(P<0.01)。脑梗死组血清BDNF水平与Ngb和MBP水平呈负相关(r=-0.654,P<0.05;r=-0.768,P<0.05),Ngb与MBP水平呈正相关(r=0.622,P<0.05)。结论 Ngb、BDNF和MBP均参与急性脑梗死的发生、发展,监测其水平变化对评估急性脑梗死的病情及预后具有重要的临床意义。Objective To observe level changes of neuroglobin (Ngb), brain-derived neurotrophic factor (BDNF) and myelin basic protein ( MBP) in patients with acute cerebral infarction, and the relationship between severity of pathogenet-ic condition and prognosis. Methods A total of 126 patients with acute cerebral infarction admitted during January 2012 and December 2015 were selected as cerebral infarction group, and 30 healthy persons taking physical examination at the same pe-riod were selected as control group. Changes of serum Ngb, BDNF and MBP levels were compared in two groups, and relation-ships between each index and degree of cerebral nerve of functional injury, infarct size and prognosis and the relationships a-mong indexes were observed. Results Compared with those in control group, Ngb and MBP levels were significantly in-creased (P〈0. 01), while BDNF level was significantly decreased in cerebral infarction group (P〈0. 01). In cerebral in-farction group, serum Ngb and MBP levels were increased (P〈0. 01), while BDNF level was decreased with aggravated de-gree of neurological function and increased infarct area (P〈0. 01). In cerebral infarction group, Ngb and MBP levels were significantly decreased (P〈0. 01), while BDNF levels were significantly higher in patients with good prognosis compared with those of patients with poor prognosis ( P 〈0. 01 ) . Serum BDNF levels were negatively correlated to Ngb and MBP levels (r= -0. 654, P〈0. 05;r= -0. 768, P〈0. 05), while Ngb levels were positively correlated to MBP levels in patients with acute cerebral infarction (r= 0. 622, P〈0. 05). Conclusion Ngb, BDNF and MBP are involved in pathogenesy and devel-opment of acute cerebral infarction, and therefore it is significant to detect the levels for evaluation of pathogenetic condition and prognosis.
