弥漫大B细胞淋巴瘤组织中Bcl-2、NF-κB、c-Myc蛋白表达及临床意义

摘　　要：目的 探究Bcl-2、NF-κB、c-Myc蛋白在弥漫大B细胞淋巴瘤（diffuse large B-cell lymphoma,DLBCL）组织中的表达及其临床意义。方法 选取2014年7月至2015年10月于本院确诊的35例DLBCL患者的淋巴瘤组织纳入试验组,另选取本院同期35例健康志愿者的淋巴组织纳入对照组。采用免疫组织化学法检测DLBCL患者淋巴组织中Bcl-2、NF-κB、c-Myc蛋白表达情况,统计分析其与DLBCL患者临床病理特征之间的关系。结果 试验组患者淋巴组织中Bcl-2、NF-κB、c-Myc蛋白阳性表达率分别为74.3%、60.0%、65.7%,对照组分别为42.8%、34.2%、37.1%,两组比较差异显著（P〈0.05）;Bcl-2、NF-κB、c-Myc在不同性别、不同临床分期的DLBCL患者中的阳性表达率无显著差异（P〉0.05）。在国际标准化预后指数（international prognostic index,IPI）评分3~4分、结外受侵、肿瘤完全缓解的DLBCL患者中Bcl-2、NF-κB、c-Myc蛋白的阳性表达显著高于IPI评分0~2分、非结外受侵、肿瘤未完全缓解的患者（P〈0.05）。结论 DLBCL患者淋巴组织中Bcl-2、NF-κB、c-Myc蛋白的表达水平较高,且常见于结外受侵、肿瘤未完全缓解及IPI评分较高的患者。Objective To explore the expression and clinical effect of Bcl-2, NF-κB, c-Myc protein in diffuse large B-cell lymphoma （DLBCL）. Method 35 cases of DLBCL patients＇ lymphoma tissue were selected as experimental group, 35 cases of healthy persons＇ normal lymphoid tissues were selected as control group. The protein expression levels of Bcl-2, NF-κB, c-Myc were detected by immunohistochemistry. The correlation of protein expression levels of Bcl-2, NF-κB, c-Myc among DLBCL patients, the relationship among protein expression levels of Bcl-2, NF-κB, c-Myc and the clinicopathological characteristics of DLBCL patients were analyzed. Result The positive expression rates of Bcl-2, NF-κB, c-Myc protein in the lymph tissues of experimental group patients were 74.3%, 60.0% and 65.7%, which were significantly higher than those of control group （42.8%, 34.2%, 37.1%） （P 〈 0.05）. There were no significant differences in the expression levels of Bcl-2, NF-κB, c-Myc protein in DLBCL patients with different genders and clinical stages （P 〉 0.05）. Positive expression rates of Bcl-2, NF-κB, c-Myc protein in patients with international prognostic index （IPI） 3 - 4 score, external invasion and tumor complete remission tumor were significantly higher than those in patients with IPI score 0 - 2, the non-nodal invasion, tumor incomplete remission. Conclusion Expression levels of Bcl-2, NF-κB, c-Myc protein are higher in DLBCL patients, and are commonly seen in patients with high IPI score, tumor invasion and tumor incomplete remission.
