改制腰穿针行泪道置管的临床经验

摘　　要：目的总结分析改制腰穿针泪道置管术的临床经验。方法对2013年4月至2015年2月在成都军区总医院眼科行改制腰穿针联合聚酯线硅胶管倒U形泪道置管术28例、32只眼，其中单纯鼻泪管阻塞9只眼，单纯下泪小管或泪总管阻塞15只眼，泪道阻塞合并轻度泪囊炎症者8只眼；取管后随访0．5。19月，记录手术操作情况，观察手术效果和并发症情况。结果术中探通时自觉非阻塞区泪道黏膜损伤或形成可疑假道者7只眼；取管后自觉流泪症状改善且冲洗通畅21只眼（65．6％）。结论泪道置管术中，下鼻道充分扩张、避免医源性泪道损伤、熟悉引导针管的位置对手术顺利进行和提高成功率进行至关重要。Objective To sum up the experience of intubation of silicone lacrimal drainage tube by modified lumbar puncture needle. Methods Retrospective reviewed 28 cases that underwent intu- bation of silicone lacrimal drainage tube by modified lumbar puncture needle, including 8 simple na- solacrimal duct obstruction, 15 simple lower lacrimal canaliculi or common canaliculus obstruction, 8 mild chronic dacryocystitis. Follow-up time was 0.5-19 months after taking off silicone tubes. The operation and complication was recorded. Results Seven eyes had mucosal injury of lacrimal duct or false passages. Improvement of epiphora was achieved in 21 eyes at last follow-up. Conclusions These aspects are important for operations of intubation of silicone lacrimal drainage tube： sufficient- ly expansion of inferior nasal meatus, avoidance of mucosal injury of lacrimal duct, knowing well the position of guided tube.
