鼻咽癌组织Raf-1的表达与鼻咽癌放疗敏感性的关系探讨

摘　　要：目的探讨鼻咽癌组织Raf-1的表达与鼻咽癌放疗敏感性的关系。方法选取72例鼻咽癌患者,根据患者放疗疗效分为放疗敏感患者37例(放疗敏感组),放疗抵抗患者(放疗抵抗组)35例,采用免疫组化染色检测两组Raf-1蛋白表达情况。结果两组患者性别、年龄、临床分期、组织分型及分化类型比较差异无统计学意义(P﹥0.05);放射敏感组Raf-1蛋白阳性表达率为29.73%,低于放射抵抗组的91.43%(P﹤0.05);放射敏感组Raf-1蛋白表达总体强度明显弱于放射抵抗组(P﹤0.05);Raf-1蛋白阳性表达预测鼻咽癌患者放疗抵抗的灵敏度为97.14%,特异度为70.27%,阳性预测值为75.56%,阴性预测值为96.29%。结论 Raf-1蛋白阳性表达率和表达强度与鼻咽癌放疗抵抗有关,对预测鼻咽癌放疗疗效有一定价值。Objective To investigate the relationship between the expression of Raf-1 in nasopharyngeal carcinoma （NPC） and the sensitivity to radiotherapy. Method 72 cases of NPC were included in the analysis, and were stratified as high radiosensitivity group （n=37） and low radiosensitivity group （n=35）, in which the expression of Raf-1 protein was detected by immunohistochemical staining. Result There was no significant difference in gender, age, clinical stage, histological type and differentiation between the two groups （P〉0.05）;The positive expression rate of Raf-1 pro-tein in the high radiosensitivity group was 29.73%, which was lower than that in the low radiosensitivity group at 91.43%, and the difference was statistically significant （P〈 0.05）; The overall expression intensity of Raf-1 protein in the high sensitivity group was significantly lower than that in the low sensitivity group （P〈0.05）; The positive expres-sion of Raf-1 has sensitivity of 97.14%, and specificity of 70.27%in predicting the radiation resistance in NPC patients, with the positive predictive value being 75.56%, and negative predictive value being 96.29%. Conclusion The positive expression rate and expression intensity of Raf-1 protein are related to the radiation resistance of nasopharyngeal carcino-ma, which is useful in predicting the curative effect of radiotherapy for nasopharyngeal carcinoma.
