膀胱镜下开窗术治疗小儿输尿管囊肿疗效分析

摘　　要：目的通过膀胱镜下开窗术治疗小儿输尿管囊肿,观察临床疗效。方法回顾性分析2010—2016年采用膀胱镜下开窗术治疗小儿输尿管囊肿19例,病例经B超、CT平扫及强化、IVU等检查确诊,收集信息包括术前症状、手术年龄、性别、手术时间、术中出血量、住院天数、手术前后输尿管扩张及肾积水的变化,术后行B超及IVU复查。结果本组患者均一次手术切开囊肿,随访6月~36个月,复查未见囊肿复发,术后肾积水及输尿管扩张情况较术前均明显改善（P〈0.05）,2例异位输尿管囊肿合并重复肾及重复输尿管患儿术后出现膀胱输尿管返流,再次行重复肾及重复输尿管切除术。结论 B超结合CT、IVU检查可以明确诊断,经囊肿开窗术治疗小儿单纯输尿管囊肿疗效确切,对术前肾积水及输尿管扩张改善明显,是一种安全有效的微创疗法。Objective To evaluate the efficacy of fenestration through cystoscope for treating ureteroceles in children. Methods Retrospective analysis of clinical data of 19 patients with ureterocele was made in the study from 2010-2016. Transurethral fenestration to treat ureterocele in children,were diagnosed and reviewed by Ultrasonography,CT and intravenous urogram. We collected information including preoperative symptoms,age at transurethral incision,age,gender,duration of operation,amount of bleeding,hospital day,improvement of hydronephrosis and ureterectasia. Results Ureteroceles disappeared after procedure in all children, were followed-up for 6 months to 3 years. In most of the patients symptoms relieved significantly,no cases were found to have recurrence after operation,postoperative hydronephrosis and ureterectasia were significantly improved（ P〈0.05）. 2 cases of duplex system ureteroceles with vesicoureteral reflux required second surgeries. Conclusions Ureteroceles can be diagnosed definitely by Ultrasonography,CT and intravenous urogram. To improve the hydronephrosis and ureterectasia,Fenestration through cystoscope for treating ureteroceles is secure,efficient and minimal invasive procedure for children with ureterocele.
