摘  要:母乳是最适合人类新生儿的营养品,母乳中含多种低聚糖,具有调节肠道菌群平衡、调节肠道p H值、促进矿物质吸收的功效。目前,对于低聚糖的功能评价主要集中在胃肠道。新生儿坏死性小肠结肠炎(NEC)近年来有发病率逐渐上升的趋势,母乳喂养婴儿的低发病率和病死率被认为与母乳低聚糖有关,但目前仍没有明确试验表明,母乳低聚糖是否具有缓解该疾病的功效。基于低聚糖的功能,本试验通过动物模型对新生儿坏死性小肠结肠炎进行模拟,对2日龄的SD大鼠乳鼠进行连续3d的灌胃和冷刺激进行造模,同时灌胃母乳和牛乳的低聚糖提取物来观察疾病症状是否有所缓解。结果显示:灌胃牛乳低聚糖组的小鼠死亡率为77.7%,灌胃母乳低聚糖组的小鼠死亡率为44.4%,差异显著;灌胃牛乳低聚糖组组织病理切片评分3.34±0.30,而灌胃母乳低聚糖组回盲肠部组织病理切片评分2.75±0.29,并且经苏木精-伊红(HE)染色后观察发现,母乳喂养对照组的回肠结构完整,绒毛顶端略微受损,固有层及黏膜下层血管无扩张,无炎性细胞浸润且肌层完整。本研究表明,母乳低聚糖对新生儿坏死性小肠结肠炎有较明显的缓解效果。Breast milk is the most suitable food for human neonatal nutrition. There are a variety of oligosaccharides in breast milk, having the function of regulating intestinal flora balance, adjusting the pH of intestinal, and promoting the absorption of minerals. At present, the evaluation of functions of milk oligosaccharides have mainly concentrated on the gastrointestinal tract. Neonatal necrotizing enterocolitis (NEC) is a kind of intestinal disease with increasing incidence among infants in recent years. It is believed to be related to the consumption of infant formula among infants. Based on the protective effect of human milk oligosaccharides, this experiment simulated the disease in an animal model. We used intragastric administration and cold stimulation to trigger the inflammation on SD baby rats for three consecutive days. We used human and bovine milk oligosaccharides extract to observe whether the symptoms are relieved. Compared to the groups treated with bovine milk oligosaccharides which had a mortality of 77.7%, while the mortality rate of groups treated with human milk oligosaceharides significantly reduced to 44.4%. Also, compared with the pathology score of groups treated with bovine milk oligosaceharides which was 3.34~0.30, the group treated with human milk oligosaccharides was much lower than other groups. Futhermore, after hematoxylin-eosin staining, the images about groups treating with human milk oligosaccharides have complete ileum structure with the top of the villus slightly damaged. And there is little lamina propria and submucosa vascular expansion, no inflammatory cell infiltration and has intact muscular layer. All the results indicated that mice gavaged with human milk oligosaccharides had better effect on relieving the severity of neonatal necrotizing enterocolitis, but the mechanism remains to be further studied.
