多焦点人工晶体屈光度测量准确性的研究

摘  要:目的通过角膜曲率计和角膜地形图对白内障手术患者术前人工晶体角膜曲率值进行测量,同时对术后患者晶体屈光度误差进行对比分析,探究不同方法对多焦点人工晶体屈光度准确性的影响。方法收集通化市五三一医院2014年3月-2015年3月行白内障超声乳化摘除及人工晶状体植入术的白内障患者100例(135眼),术前分别用人工角膜曲率计和角膜地形图测量患眼眼轴长度和角膜曲率值,并采用SPK/Ⅱ进行计算人工晶体屈光度数,并进行对比及统计学分析;对患者进行术后随访3个月,检测患者患眼实际屈光状态,并对术前计算所得理论屈光度值和术后实际屈光度值进行对比及统计学分析。结果对比人工角膜曲率计及角膜地形图两种方法测得患眼角膜曲率值,差异无统计学意义(P>0.05)。白内障患者术后3个月患眼实际屈光度与术前预测理论屈光度差值的均值差异无统计学意义(P>0.05)。结论使用人工角膜曲率和角膜地形图两种方法测量白内障患者患眼角膜曲率无明显差异,对患者术后3个月患眼屈光误差均无明显影响。Objective To compare the corneal curvature measured in determining the power of intraocular lens by two kind devices,keratometry and corneal topographer. Methods The corneal curvature of 135eyes of 100 cases with cataract were measured using keratometry and corneal topographer. Preoperative using artificial keratometer and corneal topography measurement of eye axial length and corneal curvature value,and calculated by the SPK/II IOL diopter,and comparative analysis and statistics;3 months postoperative patients were followed up and detected in eyes with actual refraction. The calculated theoretical value and actual postoperative refraction diopter values were compared and statistically significant. Results There were no statistically significant difference among the measuring data from 2 different devices on the items of maximum,minimum and mean values of corneal curvature(P〉0.05). At 1 month after surgery,there is no significant difference in refraction(P〉0.05). Conclusion Two methods are similar in measuring central corneal curvature,At 1 month after surgery,there is no different in refraction.
