Lucentis联合改良格栅样光凝治疗糖尿病黄斑水肿的疗效

摘　　要：目的探讨雷珠单抗玻璃体腔注射联合改良格栅样光凝治疗糖尿病黄斑水肿(DME)的疗效。方法非增殖期糖尿病视网膜病合并黄斑水肿患者40例(40眼)随机均分为两组:A组采用单纯改良黄斑格栅样光凝治疗;B组采用雷珠单抗玻璃体腔注射联合改良格栅样光凝治疗。治疗后2周和2个月随访,比较两组治疗后最佳矫正视力、眼压和黄斑中心厚度(CMT)变化。结果治疗后2周和2个月,A组和B组CMT均较治疗前下降(P<0.05),且B组CMT下降更明显(P<0.05)。治疗后2周和2个月,B组视力提高患者的比例高于A组(P<0.05),A组和B组眼压较治疗前均无显著改变(P>0.05)。两组治疗过程中均未发生明显不良反应。结论雷珠单抗联合改良格栅光凝治疗非增殖期DME较传统单纯激光治疗更为有效。Objective To explore the efficacy of lucentis combined with modified grid photocoagulation in the treatment of diabetic macular edema（DME）.Methods Forty patients（40eyes）with non-proliferative diabetic retinopathy combined with macular edema were treated by modified grid photocoagulation（group A,40cases）or lucentis intravitreal injection combined with modified grid photocoagulation（group B,40cases）.The patients were followed up two weeks and two months after treatment.The changes of the best corrected visual acuity（BCVA）,intraocular pressure（IOP）and central macular thickness（CMT）before and after treatment were recorded and compared between two groups.Results Compared to before,the CMT in both groups was significantly decreased after treatment（P〈0.05）,which was decreased more in group B than that in group A（P〈0.05）.The percentage of the patients with BCVA increase after treatment was higher in group B than that in group A（P〈0.05）.Compared with before,there were no significant differences of IOP two weeks and two months after treatment in both groups（P〉0.05）.No obvious complications were seen during treatment in both groups.Conclusion Lucentis combined with modified grid photocoagulation is more effective than modified grid photocoagulation alone in the treatment of nonprotiferative DME.
