胃癌患者凝血功能与病理特征的关系探讨

摘　　要：目的探析胃癌患者外周血各凝血指标水平变化的意义。方法分析2012年4月至2015年3月在我院接受治疗的101例胃癌患者的临床资料。比较胃癌患者不同浸润深度、不同临床分期、不同分化程度以及不同病理类型间的活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、凝血酶原时间(PT)、血浆纤维蛋白原(FIB)、血小板计数(PLT)、D-二聚体(D-D)的水平变化。结果胃癌患者不同浸润深度组间,黏膜组患者的FIB、PLT的水平明显低于肌层组(P<0.05);而黏膜组和肌层组间的APTT、TT、PT、D-D水平无统计学差异(P>0.05)。PT、FIB、D-D、PLT在胃癌不同临床分期间差异显著(P<0.05);而APTT、TT水平无显著差异(P>0.05)。胃癌的分化程度越高,TT水平则越高,PLT水平则越低,TT、PLT在不同分化程度间的水平差异显著(P<0.05),而APTT、PT、FIB、D-D水平无显著差异(P>0.05)。胃癌患者各病理类型间的各凝血指标水平均无显著差异(P>0.05)。结论胃癌患者的凝血指标与其浸润深度、临床分期、分化程度相关。Objective To analyze the detective significance of the changes of the levels of coagulation indexes in patients with gastric cancer. Methods Clinical data of 101patients with gastric cancer received treatment at our hospital from April,2012 to October,2015 was retrospectively analyzed. The levels of APTT,TT,PT,FIB,PLT and D-D in the different depth of invasion, clinical stage, differentiation degree and pathologic type of patients with gastric cancer were compared. Results In the different depth of invasion,the levels of FIB,PLT of patients in mucosal group were obviously lower than those in muscular layer group（P〈0.05）,and the levels of APTT,TT,PT,D-D in mucosal group and muscular layer group had no statistical difference （P〉0.05）. The levels of PT,FIB,D-D,PLT in different clinical stages of gastric cancer had statistical differences （P〈0.05） ,but the levels of APTT,TT had no statistical difference （P 〉0.05）. The higher the differentiation degree of gastric cancer,the higher the level of TT was,and the lower the level of PLT was. The levels of TT and PLT different differentiation degree had statistical differences （P〈0.05）, but the levels of APTT, PT, FIB,D-D had no statistical difference （P〉0.05）. The coagulation index in the different pathologic type of patients with gastric cancer had no statistical difference （P〉0.05）. Conclusion the coagulation indexes of patients with gastric cancer have correlation with the depth of invasion, clinical stage, differentiation degree.
