免疫固定电泳及免疫球蛋白定量在多发性骨髓瘤诊断和分型中的应用

摘　　要：目的探讨免疫固定电泳和免疫球蛋白定量在多发性骨髓瘤诊断和分型中的应用。方法采用血清蛋白电泳技术、免疫固定电泳技术、免疫比浊法对87例多发性骨髓瘤患者进行分析。结果 87例患者血清蛋白电泳77例检出M蛋白,阳性率为88.5%,免疫固定电泳均为阳性,免疫分型IgGκ29例,占33.3%;IgGλ22例,占25.3%;IgAκ10例,占11.5%;IgAλ12例,占13.8%;IgMκ4例,占4.6%;IgMλ3例,占3.4%;κ轻链2例,占2.3%;λ轻链5例,占5.7%;同时相应类型的免疫球蛋白和轻链增高伴其它组分降低。结论免疫固定电泳和免疫球蛋白定量对多发性骨髓瘤的诊断和分型具有重要的临床应用价值。Objective To investigate the application of immunofixation electrophoresis（IFE）and immunoglobulin quantitative in diagnosis and clinical typing of multiple myeloma（MM）.Methods 87 MM patients are detected serum protein electrophoresis,IFE and immunoturbidimetry.Results Among 87 cases of MM patients,77 cases for serum protein electrophoresis test were M protein,the positive of rate was 88.5%.IFE test for all 87 cases were positive.Moreover,In cases of 87 MM patients,there were 29 cases of IgGκ,22IgGλ,10IgAκ,12IgAλ,4IgMκ,3IgMλ,2κlight chain and 5λlight chain.Furthermore,corresponding type of immunoglobulin and light chain increased,accompanying with other type of immunoglobulin decreased.Conclusion IFE has important value on clinical application in diagnosing and clinical typing of MM.
