肾上腺皮质肿瘤中E-cadherin、MMP-2及MMP-9的表达及意义

摘　　要：目的探讨上皮型钙黏蛋白(E-cadherin)、基质金属蛋白酶(MMP)-2、MMP-9在肾上腺皮质肿瘤中的表达及其临床意义。方法免疫组织化学技术检测55例肾上腺皮质肿瘤(15例肾上腺皮质癌和40例肾上腺皮质腺瘤)及10例正常肾上腺组织中MMP-2、MMP-9及E-cadherin的表达。结果 MMP-2、MMP-9及E-cadherin在肾上腺皮质癌组织中的阳性表达率分别为80%(12/15)、66.7%(10/15)和20%(3/15),在肾上腺皮质腺瘤中为27.5%(11/40)、25%(10/40)和52.5%(21/40),而在正常肾上腺组织中则为0%(0/10)、10%(1/10)和100%(10/10)。MMP-2和MMP-9在肾上腺皮质癌中的表达较腺瘤组织及正常肾上腺组织高,而Ecadherin的表达则相反,差异均有统计学意义(P<0.05)。E-cadherin、MMP-2及MMP-9与肾上腺皮质肿瘤患者的年龄、性别、肿瘤大小无关(P>0.05),但与肾上腺皮质癌中TNM分期、是否有淋巴结转移有关(P<0.05)。肾上腺皮质肿瘤中E-cadherin与MMP-2、MMP-9的表达呈负相关。结论 E-cadherin、MMP-2及MMP-9在良恶性肾上腺皮质肿瘤中呈差异性表达,E-cadherin表达的减弱与MMP-2及MMP-9表达的增强均与肿瘤的形成和转移有一定的联系。Objective To investigate the clinical implication of the expression of E - cadberin, MMP -2 and MMP - 9 in adrenocortical tumor. Methods The expression of E - cadherin, MMP - 2 and MMP9 in l0 normal adrenal tissues（NAG） ,40 Adrenocortical adenoma tissues （ACA） and 15 adrenocortical cancer tissues （ACC） were detected by immunohistochemistry. Results The positive rates of E - cadherin, MMP - 2 and MMP - 9 in adrenocortical cancer tis- sues, adrenocortical adenoma tissues and normal adrenal tissues were 20% , 52.5% , 100% ,80% , 42.5% , 0% and 100% , 65% , 10%. There were significant differences among the three groups. A negative correlation was observed be- tween E - cadherin and MMP - 2 expression in adrenocortical tumor （ including ACC group and ACA group, P 〈 0. 05 ）. MMP - 2 was positively correlated with MMP - 9. The reduced expression of E - cadherin and increased rate of MMP - 2 and MMP - 9 were closely associated with TNM staging and lymph metastasis of adrenocortical cancer （ P 〈 0. 05 ）. Conclusion The expression of E - cadherin, MMP - 2 and MMP9 in benign and malignant adrenal tumors is different. The down - expression of E - cadherin and overexpression of MMP - 2 and MMP - 9 were associated with tumor formation and metastasis. This provides a theoretical basis for early diagnosis of adrenal cortical carcinoma and distant metastasis and is a potential therapeutic target for clinical study.
