前列腺癌患者中性粒细胞与淋巴细胞比值检测的意义

摘　　要：目的探讨中性粒细胞和淋巴细胞比值（neutrophil-to-lymphocyte ratio,NLR）对前列腺癌（prostate cancer,PCa）分期和分级的价值。方法对180例PCa患者进行外周血细胞计数检查,计算NLR值,比较不同危险度、不同侵犯范围、不同血清PSA水平、不同病理分期和是否存在骨转移患者的NLR值,探讨PCa患者NLR值与危险度、侵犯范围和血清PSA水平的相关性。结果随着危险度的增加、肿瘤侵犯范围的扩大和血清PSA水平的增加,NLR值和高NLR率均呈增加趋势（P〈0.05,P〈0.01）;存在骨转移及TNM分期Ⅲ~Ⅳ期患者NLR值分别高于无骨转移和TNM分期Ⅰ~Ⅱ期患者（P〈0.01）。相关性分析显示PCa患者NLR值与危险度、侵犯范围、血清PSA水平均呈正相关（P〈0.05）。结论 NLR值与血清PSA水平、肿瘤侵犯范围及危险度有一定的相关性,可作为PCa分期和分级的有效指标。Objective To investigate the value of the prognosis of neutrophil-to-lymphocyte ratio（NLR）in patients with prostate caner（PCa）. Methods 180 patients with PCa underwent peripheral blood cell count,the NLR value was calculated,the risks,scope of infringement,serum prostate specific antigen（prostate-specific antigen,PSA）levels,pathological stages and whether there was a NLR value in patients with bone metastases were compared,the correlation between NLR value and risks,scope,serum PSA level in patients with PCa were investigated. ResultsNLR value and high rate of the NLR significantly increase with the increasing of risk,the extent of tumor invasion and the serum PSA level（P〈0.05,P〈0.01）.NLR values of the patients with bone metastases in TNM stageⅢ-Ⅳwere higher than those of the patients without bone metastases in TNM staging Ⅰ-Ⅱstage（P〈0.01）.Correlation analysis showed that patients with PCa NLR value and risk,scope of infringement,serum PSA levels were positively correlated（P〈0.05）. Conclusions NLR and serum PSA value,the scope of tumor invasion and the risk level have a good correlation,so it can be the effective index for the staging and grading of PCa.
