摘 要:目的探讨肠道微生态制剂对结直肠癌患者术后免疫功能及并发症的影响。方法采用随机数字表法将江苏省肿瘤医院普通外科2015年6月~2016年4月手术治疗的80例结直肠癌患者分为观察组和对照组,每组40例。对照组术前给予常规肠道准备,观察组术前给予微生态制剂,比较两组手术前后菌群的变化、炎症因子、免疫功能及并发症。结果观察组术后7 d的致病菌少于对照组,益生菌多于对照组,差异有统计学意义(P<0.05)。观察组术后7 d的C反应蛋白、肿瘤坏死因子α、白细胞介素6水平均低于对照组,CD4^+高于对照组,差异有统计学意义(P<0.05)。观察组术后伤口感染、全身炎症反应综合征、肺部感染、吻合口感染发生率均低于对照组,两组术后感染总发生率比较,差异有统计学意义(P<0.05)。结论肠道微生态制剂可减少结直肠癌患者术后菌群失衡,改善患者术后炎性状态和免疫功能,减少术后感染性并发症的发生。Objective To investigate the effect of intestinal Micro Ecological Preparations on immune function and complications in patients with colorectal cancer after operation. Methods 80 cases of colorectal cancer operation in Department of General Surgery of Jiangsu Cancer Hospital during the June 2015 to April 2016 were randomly divided into the observation group and the control group according to the random number table. Before the surgery, the control group was given routine bowel preparation, and the observation group was given intestinal Micro Ecological Preparation. Changes of intestinal flora, inflammatory factors, immune function and complications in the two groups were compared. Results The pathogenic bacteria in the observation group was less than that in the control group, and the probiotics in the observation group was more than that in the control group, the difference was statistically significant (P 〈 0.05). The level of C reactive protein, tumor necrosis factor α, interleukin 6 after 7 d of the operation in the observation group were lower than those in the control group, and CD4+ in the observation group was higher than that in the control group, the difference was statistically significant (P 〈 0.05). The incidence of postoperative wound infection, SIRS, pulmonary infection in the observation group were lower than those in the control group, and the incidence of complications in the two groups had statistically significant difference (P 〈 0.05). Conclusion Intestinal Micro Ecological Preparation can reduce the postoperative intestinal flora imbalance, and improve postoperative inflammatory state and immune function. It can reduce the incidence of postoperative infectious complications of patients with colorectal cancer.
