低频重复经颅磁刺激联合作业疗法对偏瘫型脑瘫患儿上肢功能的影响

摘　　要：目的:观察重复经颅磁刺激(r TMS)联合作业疗法(OT)对偏瘫型脑瘫(USCP)患儿上肢肌张力及运动功能影响。方法:将45例USCP患儿按电脑产生随机数字法分为对照组(n=23)和治疗组(n=22),对照组患儿接受常规康复训练及作业疗法,治疗组患儿在此基础上加用r TMS治疗。采用改良Ashworth分级(MAS)、Carroll上肢功能试验(UEFT)分别于治疗前、治疗后8周对两组患儿手功能恢复进行评估。结果:治疗前,两组患儿肘关节MAS、UEFT评分比较,差异无统计学意义(P>0.05)。治疗后,两组患儿MAS评分均低于组内治疗前,UEFT评分高于组内治疗前,差异有统计学意义(P<0.05);治疗组MAS评分低于对照组治疗后,差异有统计学意义(P<0.05),治疗组UEFT评分高于对照组治疗后,差异有统计学意义(P<0.05)。结论:r TMS结合OT可有效降低USCP患儿肌张力,提高其患肢运动功能。Objective： To explore the effects of low frequency repetitive transeranial magnetic stimulation（rTMS） and occupational therapy（OT） on upper limbs function of children with unilateral spastic cerebral palsy（USCP）.Method： Forty-five children with USCP were randomly divided into the treatment group（n=22） and the control group（n=23）.The children in both groups received routine therapy and OT, while the treatment group received rTMS additionally.All children were measured by the modified Ashworth seale（MAS） and Carroll upper extremities functional test（UEFT） before and after the treatment.Result： Before treatment, the MAS and UEFT of elbow joint in two groups had no significant differenees（P〉0.05）.Mter treatment, the MAS of both groups were significantly lower than before treatment（P〈0.05）, the UEFT of both groups were signifieant]y higher than before treatment（P〈0.05）.Tbo average MAS of the treatment group was significantly lower than the control（P〈0.05）, the treatment group＇s UEFT was significantly higher than that of the control group（P〈0.05）.Condusion： rTMS and OT can effectively reduce the muscle tension of children with USCP and improve their upper limbs motor function.
