Bcl-2、P^(53)和c-erbB-2蛋白在子宫内膜增生过长和子宫内膜癌中的表达及意义

摘　　要：目的 :了解bcl- 2、P5 3和c-erbB - 2蛋白在子宫内膜增生过长和子宫内膜癌中的存在状况及意义。方法 :用bcl-2、P5 3和c -erbB - 2抗体对 5 0例子宫内膜癌和 2 0例子宫内膜增生过长活检标本进行S -P法免疫组化检测bcl- 2、P5 3和c -erbB - 2基因蛋白的表达及与病理分级的关系。结果 :bcl- 2、P5 3和c -erbB - 2在内膜癌、非典型增生过长和单纯性增生过长中的表达分别为 6 0 %、10 0 %和 80 % ,6 8%、4 0 %和 0 % ,76 %、4 0 %和 2 0 %。除bcl- 2在内膜癌中的表达低于非典型增生和单纯性增生外 ,P5 3和c -erbB - 2在内膜癌中的表达均高于各组 ,且内膜癌与单纯性增生之间亦存在高度显著性差异 (P <0 0 1)。bcl- 2、P5 3和c -erbB - 2均随内膜癌的恶性程度增高而呈增高趋势 ,Ⅲ级阳性表达率均高于Ⅱ级和Ⅰ级 ,Ⅲ级与Ⅰ级之间存在高度显著性差异 (P <0 0 1) ,Ⅱ级与Ⅰ级之间亦存在显著性差异 (P <0 0 5 )。结论 :子宫内膜癌的发生与bcl- 2、P5 3和c -erbB - 2基因蛋白的过度表达有关系 ,bcl- 2、P5 3和c -erbB - 2之间可能有相互协同作用 ;bcl- 2基因蛋白的异常表达可能是内膜癌的早期诊断指标 :bcl- 2、P5 3和c-erbB - 2的表达反映了癌细胞的分化程度及增殖情况 ,因而可作为子宫内膜癌的诊断、鉴别诊断?Objective:To study the status and significances of bcl-2,P53 and c-erbB-2 in the human endometrial hyperplasia and carcinoma Methods:The samples from the 50 cases of the endometrial carcinoma and the 20 cases of the endometrial hyperplasia were detected with the antibodies against bcl-2,P53 and c-erbB-2 in the S-P immunohistochemistry study,and the relationships between the expression rate and pathological grades were analysed Results:The expression rated of bcl-2,P53 and c-erbB-2 in the endometrial carcinoma group were 60%,68% and 76% respectively,all but bcl-2 expression in endometrial adenocarcinoma was significantly lower than that in atypical hyperplasia(P<0 01),higher than that in the endometrial hyperplasia The higher malignant degree of the endometrial carcinoma were,The higher expression rates in bcl-2,P53 and c-erbB-2 were The expression rates of bcl-2,P53 and c-erbB-2 in grade Ⅲ were much higher than that in grade Ⅰ (P<0 01) Conclusion:The results suggest that the occurrence of the endometrial carcinoma has a close relation with the overexpression of gene proteins bcl-2,P53 and c-erbB-2;there may be a synergetic action among these genes,bcl-2 plays a role in the genesis of endometrial carcinoma especially at the initial stage Because the expression of bcl-2,P53 and c-erbB-2 can reflect the differential and proliferative degrees of tumour cells,they can be used as the indicators in the diagnosis,differential diagnosis and prognosis of the endometrial carcinoma
