男性酒依赖患者吸烟相关因素的初步调查

摘　　要：目的:调查男性酒依赖患者吸烟行为的影响因素。方法:选取男性酒依赖的吸烟患者40例为试验组和年龄与受教育程度匹配的男性非酒依赖吸烟者40例为对照组,采用吸烟原因问卷(RRSQ)和尼古丁依赖量表(FTND)评定吸烟原因及尼古丁依赖程度;用抑郁自评量表(SDS)和多伦多述情障碍量表(TAS-20)评定酒依赖吸烟患者情绪状态。结果:(1)吸烟原因问卷药理维度中的依赖(t=2.644,P=0.011)、刺激(t=5.567,P<0.001)和自动分量表得分(t=2.799,P=0.006)酒依赖组显著高于对照组。药理维度中的镇静与瘾的分量表得分组间差异无统计学意义(P>0.05);社会心理维度中享乐分量表(t=-2.134,P=0.037)酒依赖组低于对照组,心理意象和手口活动分量表得分两组间差异无统计学意义(P>0.05);(2)尼古丁依赖量表总分(t=4.570,P<0.001)酒依赖组高于对照组;(3)患者的吸烟原因及尼古丁依赖程度与社会心理因素相关,结果有统计学意义(r=0.309-0.807,P<0.05),吸烟原因及尼古丁依赖程度各维度得分越高,抑郁及述情障碍得分也越高。结论:男性酒依赖患者尼古丁依赖程度较非酒依赖吸烟者重,吸烟行为与尼古丁的药理特性密切相关,且社会心理因素不容忽视。Objective： To explore the related factors of smoking in male alcohol dependent patients.Methods： Forty male smokers with alcohol dependence and 40 age-and education-matched healthy male smokers were enrolled into our study,and Russell ＇s reasons for smoking questionnaire（RRSQ）,and Fagerstrom test of nicotine dependent（FTND） were used to evaluate the reasons of smoking and the severity of nicotine dependence respectively; Self-rating depression scale（SDS） and Toronto Alexithymia scale（TAS-20） were used to assess the emotional state of alcohol dependent patients.Results：（1） The scores of dependence（t = 2. 644,P = 0. 011）,stimulation（t = 5. 567,P〈 0. 001） and automation（t = 2. 799, P = 0. 006） in the pharmacological dimension of smoking questionnaire in alcohol dependence patients were higher than healthy controls,but the differences in sedative and addictive scores were not significant between the two groups（P〉 0. 05）. The scores of indulgence（t =-2. 134,P = 0. 037） in smoking questionnaire in alcohol dependence group were lower than controls,but the scores of psychological image and hand-mouth activity were not different between alcohol dependent patients and controls（P 〉0. 05）.（2） The scores of nicotine dependent scale in alcohol dependent patients were higher than healthy controls（t = 4. 570,P 〈0. 001）.（3） The reason of smoking and severity of nicotine dependence in alcohol dependent patients were associated with social psychological factors（r = 0. 309-0. 807,P 〈0. 05）. Patients with higher scores of nicotine dependencehad higher scores of depression and alexithymia.Conclusion： The severity of nicotine dependence of male alcohol dependent patients was more serious than controls. Their smoking behavior was remarkably correlated with the pharmacological properties of nicotine, and the psychosocial factors were also significant.
