摘 要:目的:建立大鼠放射性口腔黏膜炎模型,观察并分析黏膜组织炎症变化过程。方法:选择SD大鼠45只,随机分为9组:空白对照组;实验组8组均用^(60)Coγ射线以35 Gy剂量单次照射口鼻部。观察大鼠口腔黏膜状况,对炎症期间拒食的大鼠进行灌注流食喂养。分别于放射后1、3、5、7、10、14、21及28 d处死1组实验组大鼠,对照组于28 d处死,取口腔黏膜组织进行HE染色和光镜下观察。结果:放射后5~6 d,大鼠口腔黏膜出现红肿伴有小面积溃疡;7~8 d小溃疡逐渐融合成大面积溃疡,并持续到9~13 d;14 d开始,溃疡逐渐缩小并愈合;21~28 d溃疡完全愈合。HE染色显示放射后第5 d,黏膜下层出现大量炎性细胞浸润,但上皮层连续完整;第7、10 d,上皮层崩解、脱落;第14 d溃疡底部可见新生肉芽组织及新生毛细血管;第21、28 d,溃疡处有新的上皮组织出现,基底层排列紧密。结论:经^(60)Coγ射线照射成功建立放射性口腔黏膜炎大鼠模型,炎症从5~8 d开始,第9~13 d达到高峰,14 d左右开始缓解恢复,21~28 d恢复正常。Objective:To establish a non-invasive model of radiation-induced oral mucositis (RTOM)in rats and ob- serve the dynamic process of tbe occurrence, development and healing of mncosal inflammation. Methods:After randomly di- viding 45 SD rats into 9 groups, one group was maintained as a blank control group, while each rat of the other 8 groups re- ceived an irradiation of single 35 Gy dose of 60Coγ-rays. The condition of the oral mueosal was observed everyday. Each group of experimental rats was sacrificed at the 1^st, 3^st, 5^th, 7^th, 10^th, 14^th, 21^st and 28^th day after the irradiation respectively. However,the control group of rats were sacrificed only on the 28^th day. Oral mucosa tissues of the dead rats were harvested. The samples underwent HE staining for histological observation. Results: Clinically, after 5 to 6 days of irradiation, redness and swelling of oral mucosa along with formation of small ulcers were observed. In the 7th to 8th day of irradiation, small ulcers gradually merged into large ones. From the 14^th day of the 35 Gy radiation dose delivery,the ulcers began to un- dergo gradual healing. They were found to be completely healed within 21^st to 28th days after irradiation. The HE staining showed that a large number of inflammatory cells appeared in the submucosal layer after 5 days of irradiation, but the epitheli- al lamina was observed to be intact. Fragmentation and exfoliation of epithelial layer was observed only at the 7^th to 10^th day of irradiation. New blood capillaries and granulation tissue could be observed at the bottom of the ulceration at the 14th day of ir- radiation whereas at the 21^st and 28^th day, the process of re-epithelialization was noted to have started and the basal layer ceils were arranged closely. Conclusion:A model of 60Coγ-rays irradiation induced oral mucositis in rats was established. Inflam- mation was seen to occur at day 5 to 8 of irradiation which significantly reached its peak at day 9 to 13. Lesion gradually be- gan to undergo healing
