不同种植深度Ankylos种植体边缘骨吸收的对比研究

摘　　要：目的：对比分析后牙区不同种植深度种植体边缘骨吸收差异。方法：选择2010年1月一2014年1月间后牙区接受Ankylos系统种植修复患者72例，共植入113颗种植体，根据种植深度分为3组：0mm组（平齐骨缘），1mill组（骨缘下1mm），2mm组（骨缘下2mill）。随访24-66个月，测量种植体边缘骨吸收量，并评估种植体功能负载2年后颈部边缘骨水平的变化及3组之间的差异。应用SPSS13．0软件包对数据进行统计学分析。结果：3组不同种植深度的种植体周围骨质年均骨吸收量之间对比，差异无统计学意义（P〉0．05）。功能负载2年后，0mm组边缘骨水平稳定于种植体平台水平或以上的位点占28．6％，1mm为52．3％，2mm为70．4％，3组对比差异有统计学意义（P〈0．05）。结论：在平台转移设计的Ankylos系统，种植深度低于牙槽嵴水平有利于种植体周围骨组织稳定。Objective：To evaluate the influence of implant with different implanting depth on the marginal bone in the posterior region. Methods：From Jan 2010 to Jan 2014,72 patients with 113 Ankylos implants in the posterior region were followed up for 24- 66 months. According to implanting depth, there were 3 groups including 0 mm group of the bone level, 1 mm group of 1 mm below the bone level and 2 mm group of 2 mm below the bone level. The marginal bone level were evalua- ted by means of X-ray and SPSS 13.0 software package was used for statistical analysis. Results：There was no significant difference in annual crestal bone loss among three groups （P 〉 0. 05 ）. After 2-years functional load, 28. 6% of the implant sites in 0 mm group had the level of the marginal bone stabilized at or above the implant platform, respectively 52.3 % in 1 mm group and 70. 4% in 2 mm group, and there was statistically significant difference among three groups（P 〈 0. 05 ）. Con- clusion ： The Ankylos implants implanted below the bone level might have more chance to keep the marginal bone stabilized at or above the implant platform.
