摘 要:目的本研究通过探讨胃癌组织中CD34和Ki67蛋白表达与临床病理特征的相关性,进而分析其临床意义。方法收集在新疆医科大学第一附属医院手术切除及胃镜活检病理明确诊断为胃癌的组织标本,采用Eli Vision TMplus免疫组织化学染色方法检测,其中临床资料完整的245例胃腺癌标本中CD34和Ki67蛋白表达。结果 245例胃癌组织中CD34和Ki67蛋白的阳性表达率分别为61.2%和95.1%,且呈正相关,但癌旁组织中均未见其阳性表达。CD34和Ki67蛋白表达有淋巴结转移患者显著高于无淋巴结转移患者,且与肿瘤浸润深度呈正相关,与肿瘤分化程度呈负相关;CD34表达与肿瘤大小、有无脉管神经侵犯及病理分期呈正相关。结论 D34和Ki67在胃癌组织中高表达与肿瘤细胞增殖、血管形成以及预后有关。联合检测CD34和Ki67蛋白表达有助于了解胃癌发生发展、浸润转移的机制,并可作为评估胃癌恶性生物学行为及预后的参考指标。Objective To investigate the correlation between CD34 and Ki67 protein expression and their clinicopathological parameters in gastric cancer. Methods 245 patients with gastric cancer which were confirmed through histopathology in the first Affiliated Hospital of Xinjiang Medical University were collected and analyzed. And the expression of CD34 and Ki67 in 245 gastric cancer specimens was examined through immunohistochemistry. Results The positive expression rates of CD34 and Ki67 proteins were 61.2% and 95.1% respectively with a positive correlation, but there were not obviously positive expression at adjacent tissues. CD34 and Ki67 protein expression in patients with lymph node metastasis was significantly higher than those in patients without lymph node metastasis. Positive rate of CD34 and Ki67 protein expression in invasive cancer was higher than those in less invasive cancer. But those expression was negatively correlated with tumor differentiation and higher in patients with poorly differentiated gastric cancer. Furthermore CD34 protein expression showed a correlation with tumor size with or without vascular and nerve invasion and pathologic stage. Conclusion CD34 and Ki67 protein expression in gastric cancer tissues were related with tumor cell proliferation, angiogenesis and prognosis. So CD34 and Ki67 protein expression might be associated with the development, invasion, and metastasis of gastric cancer, and they can be used to evaluate the malignant behavior and prognosis of gastric cancer.
