5种肿瘤标志物在早期胃癌淋巴结转移诊断中的应用价值

摘　　要：目的探讨癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、糖类抗原72-4(CA72-4)和糖类抗原15-3(CA15-3)单一检测和联合检测对早期胃癌(EGC)淋巴结转移的诊断价值。方法定量检测127例EGC患者、90例良性胃病患者及90例健康体检者血清中5种肿瘤标志物的含量,比较EGC淋巴结转移组与未发生淋巴结转移组中标志物的变化;分别评价单一和联合检测的差异并评价其灵敏度和特异度。结果 EGC组中CEA、CA19-9和CA72-4的水平与良性胃病组及正常对照组比较,差异均有统计学意义(P<0.05)。EGC淋巴结转移组与未转移组比较,CEA、CA19-9和CA72-4水平差异有统计学意义(P<0.05)。CEA、CA19-9和CA72-4联合检测可提高诊断阳性率。结论 CEA、CA19-9、CA72-4对EGC淋巴结转移具有较高的辅助诊断价值,其联合检测更有助于提高EGC淋巴结转移的检测效率。Objective To discuss the diagnostic value of single detection and combined detection of CEA,CA19-9,CA125,CA72-4 and CA15-3 in lymph node metastasis of early gastric cancer（ EGC）. Methods The levels of serum CEA,CA19-9,CA125,CA72-4 and CA15-3 were quantitatively detected among 127 EGC patients,90 benign gastric patients and 90 healthy persons. Their changes were analyzed between EGC of lymph node metastasis and that without lymph node metastasis. Meanwhile,the sensitivity and specificity were evaluated in single detection and combined detection. Results The levels of CEA,CA19-9 and CA72-4 significantly increased in EGC group compared with benign gastric disease group and the control group,with the differences statistically significant（ P 0. 05）. The levels of CEA,CA19-9 and CA72-4 significantly increased in EGC lymph node metastasis group compared with EGC without lymph node metastasis,and the differences were statistically significant（ P 0. 05）. The joint detection of CEA,CA19-9 and CA72-4 can improve the diagnostic positive rate. Conclusion For EGC of lymph node metastasis,CEA,CA19-9,and CA72-4 possess a high diagnostic value,and combined detection is more favorable for the determination of lymph node metastasis in EGC.
