奥氮平对脑器质性和躯体疾病所致精神障碍的疗效分析

摘　　要：目的 观察不同剂量奥氮平治疗脑器质性和躯体疾病所致精神障碍的疗效及对糖脂代谢的影响。方法 选取我院收治的脑器质性和躯体疾病所致精神障碍患者80例（2013年1月~2016年1月期间）,随机分为观察组和对照组,每组各40例。两组患者均使用奥氮平治疗,对照组使用大剂量奥氮平治疗,观察组采用小剂量奥氮平治疗,对比两组患者治疗的总有效率、血糖血脂的变化、简明精神病量表（BPRS）、总体印象量表（CGI-SI）评分的情况。结果 治疗后,观察组患者的总有效率为95.00%,优于对照组（77.50%）（P〈0.05）;且空腹血糖（FBG）、总胆固醇（TC）、三酰甘油（TG）、空腹胰岛素（FINS）、稳态胰岛素评价指数（HOMA-IR）等指标明显优于对照组（P〈0.05）;两组BPRS、CGI-SI评分存在显著差异,观察组显著低于对照组（P〈0.05）。结论 小剂量奥氮平用于治疗脑器质性和躯体疾病所致精神障碍患者的效果更好。Objective To analysis the curative effect of different doses of olanzapine on mental disorder caused by organic brain and body diseases. Methods From January 2013 to 2016 January, 80 cases of mental disorders caused by organic brain disease and body diseases in our hospital were selected. They were randomly divided into the observation group （40 cases ） and the control group （40 cases ） . The control group was received the large dose of olan- zapine treatment, the observation group was treated with small dose olanzapine treatment. The total effective rate, blood glucose and blood lipid changes, BPRS and CGI-SI scores were compared between two groups. Results After treatment, the total effective rate of the observation group was 95 %, which was better than that of the control group （77.50 % ）（P〈 0.05 ）; FBG, FINS, HOMA-IR, TC, TG of the observation group were significant better than those in the control group （P〈 0.05 ）; the observation group＇ s BPRS and CGI-SI scores were significant lower than that in the control group （P〈 0.05 ） .Conclusion Compared with the large dose treatment, the small dose of olanzapine treatment have better effect on mental disorders caused by organic brain and body disease.
