“天癸一号方”治疗抗精神病药物所致药源性闭经的临床效果

摘　　要：目的 考察“天癸一号方”治疗抗精神病药物引起的药源性闭经的临床作用。方法 选择2015年1月~2016年2月本科收治的抗精神病药物所致药源性闭经的女性初治患者共96例,随机分为治疗组（48例）和对照组（48例）。对照组应用常规的抗精神病药物和常规治疗闭经的药物,而治疗组则在此基础上加用“天癸一号方”。治疗结束后对两组患者月经状况改善的临床疗效、内分泌调节情况和精神状况等进行研究及比较。结果 治疗结束后,治疗组临床总有效率（89.6%）显著高于对照组（72.9%）（P〈0.05）;两组的泌乳素（PRL）在治疗后均有一定程度的下降,同时雌二醇（E2）、卵泡生成素（LH）等激素指标则出现上升的情况,差异均显著（P〈0.05）,而治疗组与对照组相比,治疗前后各激素指标的差值更大;在情绪指标上,两组的汉密尔顿焦虑量表（HAMA）得分差异不显著（P〉0.05）,而治疗组的汉密尔顿抑郁量表（HAMD）评分显著低于对照组（P〈0.05）。结论在常规治疗的基础上加用“天癸一号方”可以有助于改善患者的内分泌状况,对于治疗抗精神病药物导致的药源性闭经具有更为良好的临床疗效。Objective To explore the clinical effect of ＂Tiangui No. 1 Decoction＂ of patients with antipsychotic drug-induced amenorrhea. Methods 96 female patients on initial treatment with antipsychotic drug-induced amenor- rhea in our hospital during January 2015 to February 2016 were selected and assigned randomly into the treatment group （ 48 cases ） and the control group （ 48 cases ） . The control group was given conventional psychopharmacological treat- ment and the routine drug treatment of amenorrhea, while the treatment group was added with ＂Tiangni No.1 Decoc- tion＂ on the basis of the control group＇ s treatment. The comparisons were made between the two groups in terms of the clinical effect with menstrual conditions, the changes in their endocrine and emotional states. Results After the whole treatment, the total effective rate of the treatment group （ 89.6 % ） is significantly higher than that of the control group （ 72.9 % ） . In gynecological aspect, the PRL in both groups after treatment was lower than before, while the levels of E2 and LH were higher, and all the differences were statistically significant （P〈 0.05 ） . In the treatment group all the above differences was more significant than those in the control group. There was no significant difference in HAMA score between the two groups, while the treatment group had significantly lower HAMD scores than the control group. Conclusions The combination of routine therapy and ＂Tiangui No. 1 Decoction＂ may help to improve patients＇ endo- crine status, thus has a clinical effect on their amenorrhea. In addition to this, it may also help to improve patients＇ mental state.
