摘 要:目的探讨护理工作环境在临床护士职业认同感与工作满意度之间的中介作用,为临床护士管理提供依据。方法采用护理工作环境量表、护士职业认同问卷和护士工作满意度评定量表对378名临床护士进行调查,并构建结构方程模型。结果护理工作环境量表总分为(191.60±25.42)分,低于常模(P<0.01)。护士职业认同感总分为(90.05±11.42)分。护士工作满意度量表总均分为(2.82±0.36)分,低于常模(P<0.01)。相关分析显示工作环境、职业认同感与工作满意度之间均存在不同程度的正相关(P<0.01)。结构方程模型显示,职业认同对护理工作满意度有直接效应(λ=0.638,P<0.01);职业认同对护理工作环境有直接效应(λ=0.443,P<0.01);护理工作环境对护理工作满意度有直接效应(λ=0.516,P<0.01),职业认同通过护理工作环境对护理工作满意度有间接效应(λ=0.228,P<0.01);护理工作环境、职业认同分别解释护理工作满意度16.24%与13.82%的变异量。结论护理工作环境能影响护理工作满意度,提示医院护理管理者应加强护理工作环境建设,改善工作环境,从而增强临床护士职业认同感,提高护理工作满意度。Objective To explore the mediating effect of nursing work environment on professional identity and job satisfaction of the clinical nurses,in or- der to provide inference for clinical nurses management. Methods A total of 378 clinical nurses were researched using Practice Environment Scale ( PES), the Nurse Professional Identity Questionnaire (NPIQ) and Nursing Job Satisfaction Scale (NJSS). And estabish a structural equation model. Results The total score of PES was ( 191.60 ± 25.42 ) , which was lower than the norm ( P 〈 0.01 ). The total score of NP1Q was ( 90.05 ± 11.42 ). The total score of NJSS was (2.82 ± 0.36), it was lower than the norm (P 〈 0.01 ). The correlation analysis showed a positive correlation among work environment, professional identity and job satisfaction ( P 〈 0.01 ) , structural equation model showed that professional identity had a direct effect on nurses job satisfaction ( λ = 0. 638, P 〈 0. 01 ) , and the nursing work environment ( λ = 0. 443, P 〈 0.01 ), the nursing work environment had a direct effect on nurses job satisfaction ( λ = 0.516, P 〈 0.01 ). Profes- sional identity had a indirect effect on nurses job satisfaction ( λ = 0. 228, P 〈 0.01 ) through the nursing work environment, the nursing work environment and professional identity explained respectively 16.24% and 13.82% of the variance in nurses job satisfaction. Conclusion Nursing work environment can affect nursing job satisfaction, Nursing managers of the hospital should enhance nursing work environment building, improving nursing work environment, in order to en- hance professional identity feeling and improve nursing job satisfaction of the clinical nurses.
