艾司西酞普兰与舍曲林治疗抑郁症患者疗效的比较研究

摘　　要：目的:比较艾司西酞普兰与舍曲林治疗抑郁症患者的疗效及安全性。方法:58例符合CCMD-3诊断标准的例抑郁症患者随机分为艾司西酞普兰组(30例)和舍曲林组(28例)。艾司西酞普兰组患者采用可变剂量治疗;舍曲林组患者采用舍曲林治疗。两组患者的疗程均为8周。在进行治疗的第0、1、2、4、8周的周末,采用汉密尔顿抑郁量表(HAMD-17)和汉密尔顿焦虑量表(HAMA)评定两组患者的临床疗效。用治疗中出现的不良反应症状量表(TESS)及相关实验室检查评定两组患者的药物安全性。结果:治疗第8周末,艾司西酞普兰和舍曲林组患者的HAMD-17评分分别从(21.58±2.24)、(20.78±2.13)减到(6.79±3.67)、(6.94±4.30);有效率分别为86.67%(26/30)、82.14%(23/28)痊愈率为56.67%(17/30)、53.57%(15/28);两组患者的总体疗效差异无显著性(P>0.05)。1周末,两组患者的HAMD-17评分差异有显著性(P<0.05)。各时点两组患者HAMA评分无显著性差异。两组患者不良反应均较轻,且无脱落病例。结论:艾司西酞普兰和舍曲林治疗抑郁症均安全有效,艾司西酞普兰起效较舍曲林快。Objective： To compare efficacy and safety of Escitalopram and Sertraline in treatment of depression. Methods： Fifty eight patients meeting CCMD-3 criteria of depression were randomized into Escitalopram group（ n = 30） and Sertraline group（ n =28）. All the patients were treated for 8 weeks. The Escitalopram group was treated with flexible dosages of Escitalopram,while the Sertraline group was treated with Sertraline. At the 0,1st,2nd,4th and 8th weekends,the efficacy was evaluated by Hamilton depression scale-17（ HAMD-17） and Hamilton anxiety scale（ HAMA）,the adverse reactions were assessed by treatment emergent symptom scale（ TESS）,and safety was evaluated by related laboratory examinations. Results： At the 8th weekend,the HAMD scores decreased from（ 21.58±2.24） and（ 20.78±2.13） to（ 6.79±3.67） and（ 6.94±4.30） in the Escitalopram group and the Sertraline group,respectively; the response rate and remission rate were 86.67%（ 26 / 30）,82.14%（ 23 / 28） and 56.67%（ 17 / 30）,53.57%（ 15 / 28） in the Escitalopram group and the Sertraline group,respectively. There was no significant difference in the total efficacy between the two groups（ P〉0.05）. At the 1st weekend,there was a significant difference in the HAMD-17 score between the two groups（ P〈0.05）;however,there were no significant difference in the HAMA scores between two group at any point in time. The adverse reactions were mild in the two groups. No patients dropped-out from the trial. Conclusions： Escitalopram and Sertraline are both effective and safe antidepressant,but Escitalopram has a rapid onset.
