P16INK4a蛋白在结直肠癌中表达及临床意义

摘　　要：目的探讨P16INK4a蛋白在结直肠癌中表达情况及临床意义。方法结直肠癌患者64例,手术切除组织标本行免疫组织化学检查,观察P16INK4a蛋白表达情况,分析P16INK4a蛋白表达与临床病理特征间的关系。结果P16INK4a蛋白阴性表达12例(18.75%)、弱阳性表达4例(6.25%)、阳性表达25例(39.06%)、强阳性表达23例(35.94%),48例表达上调(75.00%);P16INK4a蛋白表达上调比例在男性(70.00%)与女性(79.41%)、高分化癌(71.74%)与低分化癌(83.33%)、临床分期≤Ⅰ期(84.62%)与临床分期≥Ⅱ期(72.55%)上差异无统计学意义(P>0.05),在无淋巴结或远处转移(88.89%)与有淋巴结或远处转移(57.14%)上差异有统计学意义(P<0.05)。结论P16INK4a蛋白表达上调是结直肠癌的重要免疫表型特征,且无淋巴结或远处转移者表达高于有淋巴结或远处转移者,可作为结直肠癌诊断及转移能力评估的重要分子靶标。Objective To investigate the expression of P16INK4 a protein in colorectal carcinoma and its clinical significance.Methods Sixty-four cases of primary colorectal carcinoma underwent immunohistochemic examination to observe the expression of P16INK4 aprotein and analyze the relationship between the expression of P16INK4 aprotein and clinicopathologic features.Results Among 64 cases of colorectal carcinoma,P16INK4 aexpression was negative in 12cases（18.75%）,weak positive in 4（6.25%）,positive in 25（39.06%）and strong positive in 23（35.94%）,and were up-regulated in 48（75.00%）.There were no significant differences in the up-regulation rate of P16INK4 aprotein between males and females（70.00%vs 79.41%）and the clinical stage of≤Ⅰ stage and≥Ⅱ stage（84.62%vs 72.55%）（P〉0.05）,and there was a significant difference between the patients with no lymphatic/distant metastasis and those with lymphatic/distant metastasis（88.89% vs 57.14%）（P〈0.05）.Conclusion The up-regulation of P16INK4 ais an important immune phenotypic feature of colorectal cancer,and the expression of P16INK4 ais higher in patients with no lymphatic/distant metastasis than that in those with lymphatic/distant metastasis.This expression pattern of P16INK4 a may become an important molecular diagnostic target for colorectal cancer,and can be used to evaluate the metastasis capacity.
