C3、C4和β_2微球蛋白水平联合检测在脓毒症患者诊断和预后评估中的应用

摘　　要：目的探讨补体C3、C4和β_2微球蛋白(β_2-MG)水平联合检测在脓毒症患者诊断和预后评估中的应用价值。方法对357例脓毒症患者的临床资料进行回顾性分析,根据患者的病情分为轻度脓毒症组和重度脓毒症组,根据28d后患者的生存状况分为生存组和死亡组,分别检测各组患者血清中补体C3、C4和β_2-MG的水平并与180例健康对照者(健康对照组)进行对比,分析血清中补体C3、C4和β_2-MG联合检测在脓毒症诊断和预后评估中的应用价值。结果与健康对照组相比,脓毒症组患者补体C3和C4水平显著降低,且重度脓毒症患者C3和C4显著低于轻度脓毒症患者,而β_2-MG水平与C3和C4呈相反趋势(P<0.05);C3、C4和β_2-MG联合检测诊断脓毒症的曲线下面积(AUC)为0.96,优于各项指标单独检测的AUC(0.76、0.75和0.87)。死亡组脓毒症患者C3和C4水平低于生存组,β_2-MG水平高于生存组(P<0.05);C3、C4和β_2-MG联合评估脓毒症预后的AUC值为0.91,优于各项指标单独评估的AUC(0.67、0.71、0.85)。结论 C3、C4和β_2-MG联合检测在脓毒症诊断和预后评估中有重要价值,可在临床中广泛应用。Ojeive o exploe he ppliion vlue of omined deeion of 3,4 nd β2-MG level in he dignoi nd pognoi evluion of epi pien.
