新疆维、汉两民族胃印戒细胞癌临床病理特征及预后影响因素

摘　　要：目的:分析新疆地区维吾尔族与汉族胃印戒细胞癌(SRC)临床病理与生活情况之间的差异,并对维族胃SRC的预后影响因素进行分析。方法:收集新疆医科大学附属肿瘤医院2009年1月至2015年1月收治的胃SRC患者120例,其中维吾尔族57例,汉族63例,对维、汉族患者的性别、年龄、肿瘤部位、大小、浸润深度、淋巴结转移、远处转移及术后中位进展时间、生活率进行比较。对维吾尔族胃SRC患者的临床病理指标与预后的关系进行分析。结果:两组的发病年龄、淋巴结转移、远处转移及术后中位进展时间,差异具有统计学意义(P<0.05),在性别、肿瘤部位、大小、浸润深度及术后生活率,差异无统计学意义(P>0.05)。结论:(1)与汉族相比,维族发病年龄早,淋巴结及远处转移多,术后中位进展时间早,进展风险高。(2)多因素分析显示,维族患者的性别、淋巴结N、浸润深度T是判断其预后的重要指标。Objective To analyze the differences in the clinical characteristics, pathological characteristics and survival between Uygurs and Hans gastric signet ring cell carcinoma （SRC）, and the clinicopathological data of Uygurs SRC were analyzed by COX proportional hazard model. Methods 120 clinical data of patients with gastric cancer from January 2009 to January 2015 were analyzed. The patients were divided into Uygurs and Hans gastric signet ring cell carcinoma （SRC）. The gender, age, tumor location, size, depth of invasion, lymph node and distant metastasis, the median time to progression, survival rate after radical operation of two groups were analyzed. The clinicopathological data of 57 cases with Uygurs SRC were analyzed, multivariate analysis was done by COX proportional hazard model. Results In Uygurs, the age, lymphnode metastasis, distant metastasis and the median time to progression were statistically different from Hans （P 〈 0.05）, But the gender, tumor location, size, depth of invasion and survival rates after radical surgery were not statistically different from Hans （P 〉 0.05）. Conclusion Compared with Uygurs, Hans has early age of onset, more lymph node and distant metastasis, and is prone to progression. Also, age, lymphnode metastasis and depth of invasion serve as independently prognostic indexes of Uygurs gastric SRC by COX proportional hazard model.
