埋藏缝合法对双侧完全性唇裂修复术人中重建效果的研究

摘　　要：目的探讨重度双侧完全性唇裂人中重建术的手术新方法。方法随机选择重度双侧完全性唇裂病例19例。术中在前唇设计模拟正常人中嵴的弧形切口，侧唇作肌脱套式解剖并在缝合时作对称性的埋线减张缝合。于术毕即刻及术后3～24个月观察人中重建效果。结果19例患者切口均一期愈合，其中18例上唇在动、静态时外形均较佳，术后人中呈上窄下宽外形，并有明显的人中凹及人中嵴，达到了美学的功能性人中重建效果；仅1例前唇组织太少的大龄患者效果差。结论埋藏缝合法可使重度双侧完全性唇裂达到美学的功能性人中重建效果，但对前唇组织太少的大龄患者不适用。Objective To explore the effect of the reconstruction of philtrum by strainless suture with symmetrical suture embedding for bilateral complete cleft lip. Methods 19 cases of serious bilateral complete cleft lip were randomly selected and underwent the following therapy： arc-shaped cut was designed on the anterior lip operatively; anatomical dissociation were made in the lateral lip＇s muscle and were sutured strainlessly by symmetrical suture embedding. The result was observed postoperatively. Results The incisions in all the 19 cases primarily healed. Satisfactory configurations were achieved in the upper lips either during moving or in still and all philtrums had upper-narrow and infra-wide outline with obvious concave and ridge on them in 18 cases. Only 1 elderly patients got poor treatment effect because of too little tissue in anterior lip. Conclusions Strainless suture through symmetrical suture embedding for Serious bilateral complete cleft lip can achieve a satisfactory contour of philtrums but not for the elderly patients with too little tissue in anterior lip.
