n-3脂肪酸对ESRD合并心血管病患者甲基化精氨酸水平的影响

摘　　要：目的:研究n-3脂肪酸对非血液透析的终末期肾脏病(ESRD)合并心血管疾病患者体内甲基化精氨酸水平的影响,进一步探讨n-3脂肪酸在ESRD患者发挥心血管保护作用的机制。方法:根据入选标准随机选择ESRD(以慢性肾脏疾病5期作为代表)合并心血管疾病的患者114名,在标准饮食的基础上随机选取57名给予1.4 g n-3脂肪酸干预(实验组),其余给予等量的橄榄油干预(对照组)。实验前后酶联免疫法(ELISA)检测各组患者循环中非对称性二甲基精氨酸(ADMA)、对称性二甲基精氨酸(SDMA)和L精氨酸的水平,气液色谱法检测血浆n-3脂肪酸中二十碳五烯酸(EPA)和二十二碳六烯酸(DHA)的浓度。结果:给予n-3脂肪酸干预3月后ADMA、SDMA和L精氨酸水平未见明显改变。研究对象血清中EPA和DHA的水平较实验前显著升高。结论:n-3脂肪酸对非血透的ESRD合并心血管疾病循环中甲基化精氨酸的水平没有影响,n-3脂肪酸并未通过减少ESRD合并心血管疾病患者循环中甲基化精氨酸的水平而起到心血管的保护作用。Objective：It is to investigate the effect of n- 3 polyunsaturated fatty acids on levels of methylarginine in patients of non- dialysis end stage renal disease（ ESRD） with cardiovascular disease,then in order to explicite the mechanism of preventing cardiovascular disease in ESRD. Methods：We recruited 114 non- dialysis ESRD subjects（ chronic kidney disease stage IV） with documented cardiovascular disease,subjects were divided into 1. 4 g / d n- 3 polyunsaturated fatty acids intervene group（ study group） and olive intervene group（ control group） with the same dose on the base of standardized diet,asymmetrical dimethylarginine（ ADMA）,symmetrical dimethylarginine（ SDMA） and L arginine were detected by ELISA,the constitute of polyunsaturated fatty acids were assessed by gas – liquid chromatography. Results：All study groups subjects had higher EPA and DHA concentrations than control subjects after 3 months,ADMA,SDMA and L arginine had no change between study group than control group. Conclusion：Intervention with n- 3 polyunsaturated fatty acids demonstrate no influence with ADMA,SDMA and L arginine,n- 3 fatty acid can not decrease the level of methylarginine to play a protection effect in ESRD with cardiovascular disease.
