摘  要:目的探讨RelA基因修饰供体骨髓来源的树突状细胞(dendritic cells,DC)在大鼠角膜移植免疫排斥反应中的作用。方法以SD大鼠为供体,Wistar大鼠为受体,建立大鼠穿透性同种异体角膜移植动物模型。将60只受体大鼠随机分为4组,每组15只,分别为对照组(造模前7 d尾静脉注射PBS 1 mL)、mDC组(造模前7 d尾静脉注射mDC 1 mL,共5×10~6个细胞)、imDC组(造模前7 d尾静脉注射imDC 1 mL,共5×106个细胞)及RelA-DC组(造模前7 d尾静脉注射RelA修饰的DC 1 mL,共5×10~6个细胞)。术后每天观察角膜植片免疫排斥反应情况并记录角膜植片的存活时间;术后14 d每组随机处死5只受体大鼠取其角膜作组织病理学观察。结果 RelA-DC组角膜植片存活时间为(26.2±2.3)d,较对照组(11.4±1.3)d、mDC组(9.3±1.3)d及imDC组(19.4±2.6)d均明显延长,差异均有统计学意义(均为P<0.05);imDC组的存活时间显著长于对照组、mDC组(均为P<0.05);与mDC组比较,对照组植片的存活时间延长,差异有统计学意义(P>0.05)。角膜植片组织病理学(HE染色)结果显示,对照组、mDC组角膜植片明显增厚、水肿,基质层胶原纤维排列紊乱,可见大量炎症细胞浸润以及新生血管;imDC组角膜植片呈不同程度水肿,基质层胶原纤维排列轻度紊乱,可见少量炎症细胞浸润;RelA-DC组角膜植片轻度水肿,基质层胶原纤维排列较规则,几乎没有单核细胞浸润。结论静脉输注RelA基因修饰的DC能抑制大鼠角膜移植术后免疫排斥反应的发生,明显延长角膜植片的存活时间。Objective To study the effects of ReLA gene modified dendritic ceils (DC) derived from bone marrow on rat graft rejection after penetrating keratoplasty. Methods SD rats and Wistar rats were respectively used as donors and recipients to establish allograft corneal transplantation models. The sixty recipients were randomly divided into control group ( injected PBS 1 mL via tail vein at 7 days before operation), mDC group (injected 5 × 10^6 mDC 1 mL via tail vein at 7 days before operation) ,imDC group (injected 5 × 10^6 imDC lmL via tail vein at 7 days before operation), RelA-DC group(injected 5 × 10^6 RelA-DC 1 mL via tail vein at 7 days before operation) ,15 cases in each group. Ocular anterior segment reaction was observed with slit lamp microscope every day after operation. The grafts were evaluated using the Larkin corneal rejection rating system,and the graft survival time was calculated. In each group,five recipient Wistar rats were killed at 14 days after operation,and the corneal grafts were harvested for HE. Results The survival time of corneal graft in RelA-DC group was (26.2 ± 2. 3) days, compared with control group ( 11.4 ± 1.3 ) days, mDC group ( 9. 3 ± 1.3 ) days,imDC group (19.4± 2.6 )days, was significantly prolonged, the differences were statistically significant ( all P 〈 0.05 ). The survival time of imDC group was longer than those of control group and mDC group, there were statistical differences ( both P 〈 0.05). Compared with mDC group, the survival time of control group was longer (P 〈 0. 05). The results of HE showed that the graft was highly edema, and the fiber in stroreal layer was arranged disorderly with many inflammatory cells and lot of new blood vessels in control group and mDC group, which was edema in different degree and arranged slight disorderly with few inflammatory cells infiltration in imDC group.
