摘 要:目的探究伴发下斜肌功能亢进对其主导眼选择及屈光发育的影响。方法病例回顾研究。选取2012年至2015年于我院斜视与儿童眼病中心行斜视治疗的伴下斜肌功能亢进患者117例为研究对象,分为单眼先天性上斜肌麻痹伴发下斜肌功能亢进组,外斜视伴发单眼或双眼的原发下斜肌功能亢进组以及内斜视伴发单眼或双眼的原发下斜肌功能亢进组。非参数检验分别比较各组中主导眼与非主导眼下斜肌功能亢进程度;配对t检验分别比较下斜肌功能亢进眼(或双眼亢进中较重眼)与对侧眼等效球镜度数。结果单眼先天性上斜肌麻痹伴发下斜肌功能亢进组与外斜视伴发单眼或双眼的原发下斜肌功能亢进组,主导眼的下斜肌功能亢进程度总和分别为5和52,均较对侧眼的94和81轻(均为P<0.05);内斜视伴发单眼或双眼的原发下斜肌功能亢进组主导眼与非主导眼下斜肌功能亢进程度总和分别为15和26,差异无统计学意义(P=0.056)。各组下斜肌功能亢进眼与对侧眼等效球镜度数间差异均无统计学意义(均为P>0.05)。结论下斜肌功能亢进程度较重眼多不被选择用于主导眼;下斜肌功能亢进对于屈光发育无明显影响。Objective To investigate the effects of inferior oblique muscle overaction on refractive error and dominant eye. Methods Monocular superior oblique paralysis with secondary inferior oblique muscle overaction, primary inferior oblique muscle overaction with V pattern (exotropia or esotropia) in 117 cases in our hospital from 2012 to 2015 was retropectively stuided by reviewing the medical record data. Nonparametric test was used to analyze the degree of inferior oblique muscle overaction differences between dominant eye and fellow eye. Paired t-test was used to analyze the refractive error differences between inferior oblique muscle overactive eye and fellow eye. Results In monocular superior oblique paralysis with secondary inferior oblique muscle overaction group and primary inferior oblique muscle overaction with exotropia V pattern group,the total degree of inferior oblique muscle overaction of dominant eye were 5 and 52, which were less than fellow eye (94,81 ) ( all P 〈 0.05 ). The total degree of inferior oblique muscle overaction of dominant eye and fellow eye in primary inferior oblique muscle overaction group were 15 and 26, there was no statistical difference (P = 0. 056 ). There was no statistical difference in interocular refracive error in all groups ( all P 〉 0.05 ). Conclusion The degree of inferior oblique muscle overaction is less in dominant eye, and inferior oblique muscle overaction has no effect on refractive development.
