摘  要:目的探讨游离脂肪酸(free fatty acid,FFA)危险因素与颈动脉狭窄之间的相关性。方法回顾性分析杭州市下城区朝晖街道530例脑卒中高危患者,其中男性211例,女性319例,平均年龄(71.29±7.02)岁。按照患者血清FFA水平分为第一分位数组177例,第二分位数组177例,第三分位数组176例。采用彩色多普勒超声诊断仪检查颈动脉血管狭窄程度。结果logistic回归分析显示,FFA是脑卒中患者颈动脉狭窄病变的独立危险因素(OR=1.003,95%CI:1.001~1.005,P=0.001)。第一分位数组、第二分位数组和第三分位数组体质量指数异常,血压异常,血糖异常,TG异常以及HDL异常的颈动脉狭窄形成率均高于危险因素校正后的第一分位数组。结论脑卒中高危人群颈动脉狭窄与血清FFA水平密切相关,同时提示高水平FFA合并高血压、血糖异常、血脂异常等危险因素对颈动脉狭窄的发生具有促进作用。Objective To study the relationship between serum free fatty acid(FFA)level and carotid artery stenosis(CAS)in patients at high risk of ischemic stroke(IS).Methods Five hundred and thirty patients at high risk of IS(211males and 319females)aged 71.29±7.02 years were divided into first quantile group(n=177),second quantile group(n=177)and third quantile group(n=176)according to their serum FFA level.Their CAS was detected by color Doppler ultrasonography.Results Logistic regression analysis showed that FFA was an independent risk factor for CAS in IS patients(OR=1.003,95%CI:1.001-1.005,P=0.001).The BMI,blood pressure,serum blood glucose,TG and LDL levels were abnormal in three groups.The incidence of CAS was significantly higher in second quantile group and third quantile group than in first quantile group after adjustment of LDL.Conclusion CAS is closely related with serum FFA level in papteints at high risk of IS.Elevated serum FFA,blood glucose,and blood lipid levels are the risk factors for CAS.
