局限期与广泛期小细胞肺癌患者血清乳酸脱氢酶水平及其与预后的关系

摘　　要：目的分析局限期和广泛期小细胞肺癌患者血清乳酸脱氢酶(LDH)水平及其预后的关系,为治疗提供数据参考。方法选取2013年1月至2014年12月在我院接受初次治疗的164例小细胞肺癌患者,分为局限期组76例,广泛期组88例,分别检测血清LDH,并评价疗效。通过随访记录对比局限期和广泛期患者生存时间及疗效。结果小细胞肺癌患者局限期的生存时间均明显大于广泛期,治疗后有效缓解率和总有效率明显高于广泛期,LDH水平明显低于广泛期;LDH水平正常的患者生存时间明显大于LDH水平升高的患者;LDH增高的患者肿瘤肝转移率与多部位转移率明显大于正常组。结论血清乳酸脱氢酶的检测有助于判断小细胞肺癌患者的疗效,可以作为预测患者的预后的重要指标,值得临床上推广应用。Objective To analyze the relationship between serum lactate dehydrogenase（LDH） and prognosis in patients with small cell lung cancer. Methods From January 2013 to December 2014,164 patients with small cell lung cancer who underwent radical resection of lung cancer in our hospital were selected. There were 76 cases with limited stage and 88 with extensive stage. Serum levels of LDH were determined in all patients to evaluate the therapeutic effect. Survival time and curative efficacy of the patients were compared between patients with the two stages by a follow-up record. Results The survival time was longer,the effective rate and total effective rate were higher and serum LDH was lower in patients with limited stage than those with extensive stage. Moreover, patients with normal LDH level had significantly longer survival time than those with increased LDH, The liver metastasis rate and other distant metastasis rate were significantly higher in patients with increased LDH than those with normal LDH. Conclusion The detection of ser- um LDH can help to judge the curative effect of patients with small cell lung cancer. It can also be used as an important index to predict the prognosis of the patients. Therefore, it is worthy of clinical application.
