笑氧混合气体在老年患者舒适结肠镜检查中的临床应用

摘　　要：目的评价笑氧混合气体应用于老年结肠镜诊疗的效果。方法将老年行结肠镜诊疗患者分为三组,笑气组(A组)300例,丙泊酚组(B组)300例,常规结肠镜检查组(C组)300例。比较三组镇痛效果和不良反应。结果三组镇痛效果比较差异有统计学意义(P<0.01);进一步两两比较,A组、B组镇痛效果优于C组(P<0.05),B组镇痛效果优于A组(P<0.05)。三组患者的谵妄、低血压、心动过缓、低氧血症、清醒延迟>0.5 h、头昏及乏力的发生率比较差异均有统计学意义(P<0.001),其中A组的低血压、心动过缓、低氧血症及清醒延迟率显著低于B组(P<0.001)。结论笑气应用于老年结肠镜检查止痛效果较好,安全性高,特别适于合并心、肺基础疾病的老年患者。Objective To evaluate effect of nitrous oxygen combined with oxygen in elderly patients with comfortable colonoscopy.nethods A total of 900 elderly patients underwent colonoscopy were divided into nitrous oxygen combined with oxygen group（ group A） ,propofol group（ group B） and routine colonoscopy group（ group C） with 300 cases in each group. And then analgesic effects and adverse reactions were compared between the 3 groups.Results Difference of the analgesic effects of the three groups was statistically significant（ P〈0.01 ）.Further pairwise comparison showed that analgesic effects of the group A and the group B were better than that of the group C（P〈0.05） ,and analgesic effect of the group B was better than that of the group A （ P〈 0.05 ）. Incidence rates of delirium, hypotension, bradycardia, hypoxemia, consciousness delay〉 0.5 h, dizziness and weakness of the 3 groups showed statistical significance （P〈0.001）, among which incidence rates of hypotension,tachycardia,hypoxia and consciousness delay of the group A were significantly lower than those of the group B （P〈 0.001 ）.Conclusion Nitrous oxygen combined with oxygen has good effects in colonoscopy for the elderly patients with high safety, especially suitable for those combined with heart and lung underlying diseases.
