摘 要:目的探讨细胞周期蛋白D1(cyclin D1)在子宫颈上皮内瘤变(CIN)及鳞状细胞癌中的表达及其与人乳头状瘤病毒(HPV)16 E7的关系。方法选择2013-09—2015-09内蒙古自治区赤峰学院医学院附属医院经病理确诊为子宫颈上皮内瘤和宫颈鳞癌的患者以及同期宫颈正常上皮人群作为研究对象,取其子宫颈组织制成石蜡标本。其中子宫颈上皮内瘤81例,宫颈鳞癌40例,宫颈正常上皮50例。采用即时荧光定量PCR、免疫组织化学法检测cyclin D1在三组细胞组织中的基因以及蛋白的表达情况;采用逆转录PCR、Western blot、免疫组织化学法检测cyclin D1在不同E7表达状态的的三组细胞中的表达情况。结果正常宫颈组织组cyclin D1表达均为阳性,CIN组和鳞状细胞组只有少数表达为阳性,显著少于正常宫颈组织组,且随着宫颈上皮内瘤变等级的升高cyclin D1表达量逐渐降低;E7基因在正常宫颈组织中不表达或微表达,在CIN组以及鳞状细胞癌组表达更高,以上差异均具有统计学意义(P<0.05)。结论子宫颈癌的发生受多重因素影响,HPV感染是该症发生的主要因素。随着宫颈上皮内瘤变等级的升高cyclin D1表达量逐渐降低,且E7基因的高度表达是导致cyclin D1低表达的主要原因。Objective To explore the expression features of cyclin D1 in cervical intraepithelial neoplasia(C1N) and squamous cell carcinoma and its correlation with HPV16 E7. Methods Sufferers diagnosed with cervical intraepithelial neoplasia(CIN) and squamous cell carcinoma and people with normal cervical epithelium at Chifeng College Medical School Affiliated Hospital of the Nei mongol Autonomous Region from September 2013 to September 2015 were taken as the study objects,whose cervical tissues were made into paraffin-embedded specimens.There were 81 cases with cervical intraepithelial neoplasia, 40 cases with cervical squamous carcinoma, and 50 cases with normal cervical epithe- lium.Real time fluorescence quantitative PCR and immunohistochemical method were adopted to detect the expression of cyclin D1 in genes and albumen of the three kinds of cell tissues ; Reverse transcription PCR, Western blot, and im- munohistochemical method were adopted to detect the expression of cyclin D1 in the three kinds of cell tissues with different E7 expression status.Results The expression of cyclin D1 in the normal cervical tissue group was all posi- tive, while only a little of expression in the CIN group and the cervical squamous carcinoma group was positive, which was clearly less than the normal cervical tissue group.With the rising of cervical intraepithelial neoplasias lev- els ,the expression of cyclin D1 gradually lowered.Gene E7 had no or little expression in normal cervical tissues ,but quite high in the C1N group and the cervical squamous carcinoma group, with differences of statistical significance(P〈 0.05).Conclusion The occurrence of cervical cancer has multiple influencing factors, among which HPV infection is the principal factor.With the rising of cervical intraepithelial neoplasias levels,the expression of cyclin D1 gradually lowered, and the high expression of gene E7 is a main cause leading to the low expression of cyclin D1.
