超声造影在肝癌介入干预效果评价中的应用价值研究

摘　　要：目的探讨超声造影在肝癌介入治疗疗效评价中的应用价值。方法选取接受介入治疗的肝癌患者50例(67个病灶),于治疗前、治疗1个月后行超声造影检查,并对检查结果进行分析。结果 67个病灶介入治疗1个月后,超声造影下显示27个病灶内部无增强,彻底治疗率为40.30%,直径<3cm病灶、≥3cm病灶介入治疗彻底率分别为83.33%、24.49%;40个病灶有增强,直径<3cm病灶残留病灶超声造影下以周边小团状高增强包块为主,直径≥3cm病灶残留病灶超声造影下以周边不规则细节状高增强包块为主;残留病灶治疗后增强时间较治疗前延长,增强强度与曲线下面积治疗后较治疗前下降。以超声造影为"金标准",常规超声评价肝癌介入治疗疗效的灵敏度、特异度分别为74.07%(20/27)、47.50%(19/40)。结论超声造影可显示肝癌介入治疗后残留病灶的位置、形态、范围等特点,是肝癌介入治疗后疗效评价的可靠方法。Objective To discuss utilization of contrast-enhanced ultrasonography in response evaluation of hepatocellular carcinoma（HCC）after interventional therapy.Methods The observation of 67 primary foci in 50 cases HCC Before and 1 month after treatment were performed by contrast-enhancedultrasound.Results After treatment,The contrast-enhanced ultrasonography examination showed that the residual and recurrent viable tumors have not identify enhancement in any of the remaining 27 lesions for the HCC patients after response evaluation,Thetreatment rate was40.30%,diameter〈3cm and ≥3cm lesions for thetreatment rate were 83.33% and 24.49%;After treatment,The contrast-enhanced ultrasonography examination showed that the residual and recurrent viable tumors have enhancement in any of the remaining 40 lesions for the HCC patients after response evaluation,Peripheral small cloud state High enhancement bag piece of〈3cm lesions,High enhancement surrounding small section irregular block≥3cm lesions;Residual lesions enhanced after treatment time is longer than before the treatment,enhance the strength and the area under the curve is lower than before treatment after treatment.The contrast-enhanced ultrasound for the＂gold standard＂,The sensitivity of conventional ultrasonic evaluating the effect of interventional therapy of liver cancer,specific degrees were74.07%（20/27）,47.50%（19/40）.Conclusion Contrast-enhanced ultrasonography can display the position,shape and range characteristics of hepatocellular carcinoma after interventional therapy of residual lesions Position,is a reliable method of evaluating curative effect after interventional therapy of liver cancer.
