摘  要:目的 :探讨颅底沟通性肿瘤的临床特点及其CT、MRI诊断价值。方法 :回顾性分析我院经CT、MRI和组织病理学确诊的颅底沟通性肿瘤64例,比较不同病变部位患者的一般临床资料、CT和MRI特点,以及CT和MRI对颅底沟通性肿瘤、侵犯周围骨质、侵犯周围软组织诊断符合率的差异。结果:颅底沟通性肿瘤沟通部位位于前颅窝32例(50.00%),中颅窝26例(40.62%),后颅窝6例(9.38%);等密度45例(70.31%),稍高密度13例(20.31%),高密度6例(9.38%);信号均匀13例(20.31%),不均匀41例(64.06%),混杂10例(15.62%);中等强化49例(76.56%),高度强化15例(23.44%);钙化或骨化35例(54.69%)。CT对颅底沟通性肿瘤的诊断准确率与MRI差异无统计学意义(P>0.05)。CT对颅底沟通性肿瘤侵犯周围骨质及周围软组织的诊断符合率与MRI比较差异均有统计学意义(均P<0.05)。结论:CT、MRI能准确显示颅底沟通性肿瘤的病变特征,两者在诊断准确率和符合率方面各有优势。Objective:Comparison of clinical features and diagnostic value of CT and MRI in patients with skull base commu- nicating tumor. Methods:Retrospective analysis of 64 cases with skull base communicating tumor confirmed by CT,MRI and histopathology in our hospital,compare the general clinical data, CT and MRI imaging features of patients with different commu- nication sites. To compare the diagnostic accuracy of CT and MRI in the diagnosis of skull base communicating tumor,the diagnostic accuracy of the invasion of the surrounding bone,and the diagnostic accuracy of the surrounding soft tissue. Results:32 cases(50.0%) of the anterior cranial fossa,26 cases (40.62%) of the middle cranial fossa and 6 cases (9.38%) of posterior cranial fossa;45 cases (70.31%) of equal density,13 cases (20.31%) of slightly higher density,6 cases (9.38%) of high density. 13 cases (20.31%) of homogeneous signal,41 cases (64.06%) of non-homogeneous signal,10 cases (15.62%) of mixed signal;49 cases (76.56%)of medium intensity,and 15 cases (23.44%) of high intensity;35 cases (54.69%) with calcification or ossification,29 cases (45.31%) without calcification or ossification;There was no significant difference of CT (81.25%) and MRI (92.18%) in the diagnosis accuracy of skull base communicating tumor (P〉0.05);The diagnostic accuracy in skull base com- municating tumor invading the surrounding of CT (64.06%) was lower than that of MRI (85.93%),and the difference was statistically significant (P〈0.05);The diagnostic accuracy in skull base communicating tumor invading the surrounding soft tissue of CT (73.44%) was lower than the MRI (95.31%), and the difference was statistically significant (P〈0.05). Conclusion : CT and MRI can accurately display the lesion characteristics of the skull base communicating tumor,CT and MRI have their own advantages in diagnostic accuracy and coincidence rate.
